The role:
We provide continuity across in-patient and community-based secondary NHS mental health care that is based on the principles of equality, dignity and respect and which takes into account spiritual and cultural diversity.
Within the eating disorder service, care is delivered by a multi-disciplinary team in collaboration with partner agencies. It is based on the best available evidence for practice and is planned and delivered in a manner which promotes personal choice. The clinical team works collaboratively with service users, carers and their representatives to ensure that care needs are identified and met, and through utilising group/individual therapies across community and in-patient settings, will offer a credible alternative to hospitalisation or facilitate early discharge wherever possible.
All patients will be cared for by a named individual who is tasked with working closely with their identified patients/carers to develop a participative, therapeutic relationship and deliver high-quality mental health care in an environment where patients feel valued and where confidentiality is respected.
Through a short- to medium-term case management approach, the service will provide intensive support and interventions to clients and carers prior to, during and following discharge from an in-patient episode within our partner services, with an expectation of preventing relapse and for new referrals in-patient episode of treatment.

Your Duties will include telephone enquiries from all staff groups in the NHS, patients, relatives, carers and external agencies and the management of outpatient clinics. You will be expected to direct and prioritise queries as appropriate, to ensure efficiency and effectiveness of service delivery.
You will assist in the management of the mental health patient information system (EMIS) for the team ensuring information is accurate and up-to-date and should be proficient in the use of Microsoft Office to set up documents and maintain and update databases/spreadsheets, showing competence in the use of a range of computer packages evidenced by experience or formal qualifications e.g. ECDL.
