Job overview

Care Coordination invites candidates with strong team leadership skills and with experience in administration and/or call handling to apply to join their team.  The role is for shifts to be worked across 24 hours.

The post-holder will have excellent leadership and communication skills and will be required to demonstrate a significant level of initiative in providing a support service to our nursing colleagues across a variety of teams.

You will be expected to promote a professional attitude offering an efficient and confidential service and will be liaising with a wide range of clinical staff over the telephone, face to face and via mail. You will have excellent communication skills and a flexible approach to working as part of a team focused on ensuring the needs of patients and referrers are met.

Main duties of the job

The post holder will ensure that the service is sufficiently staffed to maintain support during service operational hours, including bank holidays.

The post holder will ensure a high quality and responsive administration and call handling support to meet the needs of their service.

The post holder will work closely with other Team Leaders, Team Coordinator and/or Service Support Manager to ensure consistency of services, processes and best practice across the Adult Community Services Group.

The post holder will monitor data to ensure timeliness of input of national and contract data, whilst ensuring data quality targets for demographic collection are met.

Detailed job description and main responsibilities

Communication: The post holder will ensure a strong and robust communication process between all colleagues within the service.  They will provide leadership  to the team to ensure there is an efficient and confidential message handling service.

ICT: The post holder will have an enhanced knowledge of software programmes used within the service. They will provide training and support to staff in use of systems, and support colleagues in resolution and troubleshooting of minor iT issues.

Governance: The post holder will audit staff performance levels, challenging working practices to improve administrative functions and processes.  They will monitor, manage and investigate incident reporting ensuring all Trust policies and procedures are adhered to.

Staffing:  The post holder will develop and promote a positive culture within the Directorate to enable continuous quality and service improvement in line with the Group and organisation objectives.
