Job overview

The post holder will be expected to assist with the coordination and facilitation of the TAVI service at UHNM under the guidance and supervision of the Band 7 TAVI Clinical Nurse Specialist and Consultant Cardiologist.

Provide specialist nursing care to patients with severe aortic stenosis undergoing TAVI.

Coordinating investigations and providing educational support pre and post procedure.

Assist with the development of the TAVI service to improve the patient pathway from referral through to post‑procedure follow up, contributing to service improvement initiatives as directed by senior staff.

Participate in weekly TAVI MDT discussions for case planning, contributing nursing input while recognising senior clinical decision‑making responsibility.

Main duties of the job

  • Assist in the development of the clinical services within the TAVI service.
  • To assist with the smooth planning, implementation and running of the TAVI service.
  • Assist in the referral and management of both inpatient and outpatient case load. Assess, plan, implement and evaluate evidence based TAVI nursing care within agreed clinical pathways, escalating complex or high risk decisions appropriately.
  • Provide specialist nursing advice and clinical practice in relation to the speciality, in collaboration with the Band 7 CNS and medical team.
  • Liaise with networks within the speciality at local, national and international level to exchange and enhance knowledge and expertise.
  • Be involved in the development and implementation of outpatient clinics for patients requiring assessment, ongoing care and counselling, supporting clinics led by senior clinicians or the Band 7 CNS.

Detailed job description and main responsibilities

  • Be a source of specialist nursing knowledge regarding aortic valve disease, working within own level of competence and accessing senior advice as required.
  • Optimise the involvement of patients and carers in the planning and delivery of service.
  • Assist in the discharge process for TAVI patients, including referral to other agencies and providing educational literature
  • Liaise with professionals in the community to ensure a coordinated approach meeting the needs of patients across primary, secondary and tertiary sections.
  • Provide telephone follow up and advice within agreed guidelines, referring complex concerns to the Band 7 CNS or Consultant Cardiologist.
  • Respect confidential information obtained in the course of professional practice and upholds the principles of Caldicott and the Data Protection Act.
  • Assist in the development of the pathway for single point access for surgical and TAVI patients Consultancy/Collaboration
  • Develop and maintain others’ awareness of the role within the specialty, organisation, local networks and maintain mechanisms for contact and referral
  • Collaborate with the development of services within the TAVI team
  • Assist with the day-to-day coordination of the TAVI service under the direction of the Band 7 TAVI CNS and Consultant, ensuring flexibility to meet patient needs.
  • Demonstrate an on-going awareness of current developments within percutaneous clinical program and integrate into clinical practice recognising any implications these may have of nursing care and future education and training programmes.
  • Assist in the collection and management of TAVI patient databases for audit and service monitoring purposes; responsibility for research governance and study delivery remains with senior staff.
