As a Trust we pride our-selves on our innovative and collaborative approach to providing the highest quality, person-centred care: As part of our transition to an integrated neighbourhood model of care, we are developing clinical pathways across primary, secondary care and crisis and emergency service. Our pathways embody strong partnership working with a wide range of statutory and VCSE providers and people with lived experience. Each of the advertised post has a key role in ensuring that the community therapies pathways are high quality, effective and accessible across the adult age range and that psychosocial interventions are embedded in wider service delivery
