Job summary
We have a vacancy for a full time Band 4 Stroke Practitioner within the Community Stroke service based within the city of Nottingham.
We are looking for compassionate, enthusiastic and highly motivated individuals from a health or social care setting who have experience of working with/within multidisciplinary teams and can demonstrate an understanding of how a stroke can impact on patients and those around them.
The role requires you to be self-motivated as you will be managing your own caseload. You will be part of a large but supportive multidisciplinary team carrying out assessments, and supporting rehabilitation in peoples homes, care homes and other city venues or clinic settings. Patients seen will have had a stroke within the past 2 years.
In addition to APs, the team comprises of Administrator, Discharge Coordinator, Mental Health Nurses, Occupational therapists, Physiotherapists, Rehabilitation Support Workers, Reviewing Officers, Speech and Language therapists and Stroke Nurses.
There are good opportunities for training and development both internal and external and excellent opportunities to develop the role and to be involved in shaping the service through involvement in audits or other service development projects.

Main duties of the job
The Stroke Practitioner role is key to supporting patients to progress in their recovery following discharge from hospital to either their own home environment or to a care setting, working closely with the other members of the Community Stroke Service and feeding back any changes to the clinicians involved. The role is a very important part of the service in that it supports the qualified clinicians in implementing rehabilitation of patients who have suffered a stroke by carrying out assessments and progressing clear rehabilitation goals and care plans. They also support Rehabilitation Support Workers with their caseload management.
Stroke practitioners are required to encourage and promote self-care with individuals following a stroke, providing support and guidance, and working in partnership with patients, carers, and other agencies to ensure a person-centred approach.
There are also opportunities to run groups, support clinics, and be involved in training sessions. You will be expected to be an active member of the team including being involved in ongoing service evaluation and development such as participating in audits and other projects that will improve service delivery and patient outcomes.
You will receive regular supervision and appropriate training to develop within the role.
You will need to demonstrate effective interpersonal and communication skills. You must be able to prioritise tasks and manage your time effectively. You will need to be flexible and organised.

Job responsibilities
Job Purpose
- To work within the multi-disciplinary Community Stroke Team to provide assessment and rehabilitation for patients who have had a stroke and fit the criteria for the Stroke Service.
- To deliver an integrated rehabilitation service by providing generic assessments of social care and health needs by working across professional boundaries and within legislation, policies and procedures and core values.

Key Responsibilities
Clinical
- To undertake holistic assessment of patients needs who have been referred to the service covering all aspects of peoples lives that may have been impacted by a stroke.
- To work alongside clinicians to interpret assessment information and plan and progress their intervention using judgement skills to decide upon and recommend best course of action.
- To refer patients to external agencies and specialists when their needs are beyond the assistant practitioner role under the guidance of clinicians
- To accept responsibility for designated caseload, working independently taking an active role in promoting independence in all aspects of a persons life which may have been impacted by a stroke including personal and domestic activities of daily living, physical abilities, cognition, mental health and social, meeting the needs of the patient.
- To potentially co-ordinate and lead exercise classes for stroke clients across the community.
- To promote wellbeing, safe mobility, transfers, and effective communication. To provide instruction, advice and intervention to patients and carers in line with the multi-disciplinary team guidelines.
- To assess, provide, fit, and demonstrate use of a wide range of equipment to promote independence.
- To assess for and commission a range of adaptations in line with the multi-disciplinary team guidelines.
- To assess for and commission social care services in line with the multi-disciplinary team guidelines.
- To attend and contribute to team meetings and case discussions using effective communication.
- To maintain comprehensive and accurate records in line with employers polices
Managerial and Leadership.
- To provide support to other Assistant Practitioners, and Rehabilitation Support Workers as appropriate.
Education and Development
- To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames.
