Job summary

As a Tobacco Dependency Practitioner for Secondary Care, you will be an integral part of Smoking Cessation in the hospital, working alongside existing colleagues and partners, ensuring a medical model of care for tobacco dependency is an integrated part of routine client care by all health professionals. Ultimately this programme will lead to the most significant referral route for the overall smoking cessation service within the Borough.

Main duties of the job

The successful candidate will assess, plan and deliver smoking cessation and tobacco dependency support to patients at the hospital bedside, ensuring seamless follow-up support in the community after discharge. Working under the line management of the Project Lead and Head of Service, and as part of the secondary care team under an honorary contract, you will deliver evidence-based stop smoking interventions in line with NICE and NCSCT guidance through one-to-one, group, telephone and online support. The role involves motivating and empowering clients to address tobacco addiction using behaviour change techniques, helping them set realistic goals and providing ongoing support to prevent relapse. You will work closely with hospital clinical teams and external partners to embed smoking cessation into routine care, ensure clients have timely access to appropriate pharmacotherapy on admission, and build strong relationships to strengthen referral pathways. Responsibilities include managing a caseload, maintaining accurate client records in line with ABL Health policies, engaging clients through hospital and community activity, supporting service administration, attending stakeholder and service meetings, and contributing to the development and improvement of the tobacco dependency programme. This varied role requires a motivated, organised and proactive individual who can prioritise workload effectively while supporting people to improve their health and wellbeing.

Job responsibilities

Training will be given to ensure staff / applicants are equipped to deliver effective services.

Your key responsibilities will be:

To be responsible for the assessment, planning and delivery of a programme of smoking cessation / tobacco dependency care to clients at the hospital bedside and follow up seamlessly into the community on discharge

To support with embedding smoking cessation / tobacco dependency into routine medical care

Assisting clinical teams in providing advice to help tobacco users to stop smoking or using other forms of tobacco.

Ensure tobacco dependency interventions you will be delivering comply with NICE, and NCSCT guidance.

Work cohesively with other services to ensure multi-agency working is integrated into tobacco dependency.

Identify opportunities to develop and improve services for clients.

Contribute to the development of secondary care staff.

Motivate and empower clients to make changes to address their addiction

Explain to clients the principles of the Stop Smoking Service to enable them to make informed choices

Deliver evidence-based stop smoking interventions, these include one to one, drop in, group sessions, telephone, or on-line support, these must be in accordance with the NCSCT Standard Treatment Programme.

Work with other professionals to embed smoking cessation is part of routine client care

Maximise the impact of smoking cessation on health gains and reduce health inequalities

Lead peer support group sessions for successful quitters following intervention to prevent relapse

Provide evidence-based education in all aspects of smoking cessation and behavioural change as well on- going motivational support

Work with individuals to set realistic sustainable goals

Ensure all client and service records are up to date, accurate and satisfy the ABL Health documentation policies Manage a caseload

To engage and recruit potential clients at promotional and community events around hospital sites

Provide administrative assistance to the service in terms of client contacts, appointment bookings and confirmations

Identify opportunities to develop and improve services for clients

Maintain own knowledge of evidence-based methods and integrate new research into own practice

Assist in the organisation of the workload ensuring effective and efficient communication to other staff members

To conduct stakeholder engagement, identify and strengthen referral pathways and identify potential co-working opportunities

Attend stakeholder meetings as required

Attend regular Stop Smoking Service Meetings with wider ABL Wirral team

Attend all training, internal and external to support the role

This role will be demanding as services develop and should become integrated in all settings. It will be essential that you use your own judgement to prioritise competing demands and workload effectively to specific timescales.

Duties and Responsibilities

This post will work under the line management of the Project Lead and Head of Service. The role will be varied, challenging, and rewarding as we continue to build on our existing pathways within the trust.

Work as part of the secondary care team within the boundaries of an honorary contract

To build relationships with relevant hospital personnel to ensure own expertise is utilised to influence evidence base practice is embedded and governed by hospital policies and protocols.

Work cohesively with other services to ensure smoking cessation / tobacco dependency is integrated within multi-agencies.

Participate in meetings applicable to the development of the Smoking Cessation / tobacco dependency programme within secondary care.

Ensure clients have access to appropriate pharmacotherapy as speedily as possible on admission to hospital.

Deliver smoking cessation / tobacco dependency interventions with clients at the bedside and ensure a seamless transition into ABL Wirral upon discharge.

Skills and Competencies Required

Be an experienced Specialist Stop Smoking Practitioner within the community. (Training will be given)

Experience of working with evolving and developing services

Positive and proactive outlook with the ability to motivate, engage and support others to achieve the highest standards and outcomes.

Adept in communication; A strong communicator who adapts the message to fit the needs of the audience including clients, staff, and volunteers at all levels.
