Job overview

An exciting opportunity has arisen for a band 5 named nurse to work with patients undergoing renal replacement therapy at home.

Previous experience in renal/haemodialysis is desirable but not essential, as full in-house training and support will be given. The team you will be joining is dedicated to training of both staff and patients.

You also need to be:

  • Ability to work as part of a team or independently.
  • A positive and proactive attitude.
  • A desire to deliver a high standard of care to chronically ill patients
  • Flexible working will be considered .

Undertaking adequacy tests/training/refreshers to ensure patient safety at home undertaking their treatment.

Main duties of the job

Main duties involved are of training  patients on both Peritoneal dialysis (CAPD & APD) and haemodialyis (Nx Stage machine/5008s), ensuring a high standard of care is maintained throughout

Undertake the named nurse

Carrying out Adequacy tests including PET/UKM's for Peritoneal dialysis patient .

Working as part of a team within the wider multi-disciplinary team.

Under take home visits regularly and being a  car driver is essential.

Undertake the named nurse role for patients undergoing renal replacement therapy at home.

Responsible for the teaching/ implementation and  evaluation of their treatment for a group of patients out in the community.

Actively contribute to setting and maintaining high standards of  quality nursing care.

Work collaboratively and co-operatively with others to meet the  needs of patients and their families.

Act as a clinical and professional role model,.

  • Adhere to Trust policies, procedures and clinical guidelines and ensure compliance of others. Responsible for the effective and efficient use of clinical, physical and financial resources. (Please refer to the job description/person specification for further details on the role)
