Job overview

Thistlewayte Ward is a 20-bedded acute Medicine for the Elderly ward at St Mary’s Hospital, caring for older adults with frailty, complex medical conditions and discharge needs. The ward offers a supportive learning environment in which to develop expertise in comprehensive assessment, frailty care, rehabilitation, medicines management, dementia and delirium care, multidisciplinary working, family communication and safe discharge planning.

As a Band 5 Staff Nurse, you will assess, plan, deliver and evaluate evidence-based, person-centred care for a group of patients; promote dignity, independence and wellbeing; recognise and escalate deterioration; administer medicines safely; maintain accurate documentation; and work closely with medical, therapy, pharmacy, discharge and community teams. You will also communicate effectively with patients, relatives and carers, contribute to a positive safety culture, support students and healthcare assistants, and participate in quality improvement.

Main duties of the job

• Work in accordance with the NMC Code, Trust policies, clinical guidelines and relevant legislation.
• Maintain current NMC registration, mandatory training and continuing professional development.
• Take professional accountability for own practice and seek advice or escalate concerns appropriately.
• Maintain accurate, timely and confidential patient records.
• Promote patient safety, dignity, equality, diversity and inclusion.
• Comply with safeguarding, infection prevention and control, health and safety, information governance and risk-management requirements.
• Report incidents, near misses and concerns through the appropriate Trust systems and support learning from incidents.
• Participate in clinical audit, quality improvement, service evaluation and implementation of evidence-based practice.
• Use Trust resources responsibly and contribute to a safe, efficient clinical environment.
• Communicate effectively with patients, relatives, carers and the multidisciplinary team.
