The Older Persons Assessment Unit is a 20‑bedded front‑door acute frailty assessment unit, providing rapid assessment and early intervention for older adults who present with acute or complex needs. In addition, OPAU has Same Day Emergency Care (SDEC) capacity, including 7 trolleys and 2 chair spaces, enabling us to manage patients promptly, avoid unnecessary admissions, and support effective patient flow across the hospital.

Our multidisciplinary team works collaboratively to deliver high‑quality, patient‑centred care that promotes independence, dignity, and safe, timely discharge planning.

Main duties of the job

To work according to the NMC Code of Professional Conduct and relevant professional guidelines as a named nurse or key worker (with facilitation) for a defined group of patients and take responsibility for:

  • The assessment of care and health education needs
  • The development, implementation and evaluation of programmes of care including discharge planning for each patient.

To gain experience and skills in:

  • Clinical practice
  • Facilitation and teaching
  • Management (as appropriate)

To work in accordance with the RUH Nursing Strategy and contribute towards achieving its objectives and maintain effective communication and participate in clinical supervision as appropriate and to teach and act as a facilitator/mentor/preceptor/ role model to less experienced staff.
