For this role you would need to demonstrate effective assessment, planning, monitoring and evaluation of care given to service users. You will be providing specialised, enhanced rehabilitation and recovery interventions. You will assess, plan, implement and evaluate service users care packages in line with the Care Programme.
You will work collaboratively with Service Users, Families/Carers and the Multi Disciplinary Team to provide a recovery-orientated approach and to foster autonomy and help service users to take an active part in treatment decisions and support self management. You should be motivated to work in a service that offers a recovery-orientated approach, delivering person-centred care through collaboration involvement.
