Job overview

An exciting opportunity has arisen for Band 5 Nurses to join the acute out-patient parental antimicrobial therapy (OPAT) service as part of UHD’s Hospital at Home/Virtual Ward Team. This team will also be providing care to general medical patients in the future with the development of new patient pathways being explored.

The post requires flexible working between the acute sites and the service hopes to develop in the future to provide care in patients’ homes to administer intravenous antimicrobials and perform patient observations and assessments. Therefore a valid driving license and access to a vehicle is necessary to fulfil this role.

Providing a high-quality service that is patient-centred is important to us; the successful candidate will be a highly motivated and enthusiastic nurse with a flexible, dynamic approach to patient care.

The role requires an active UK NMC Registration. Unfortunately we are not able to take any pre-NMC registered nurses in this role.

Base Location: Poole Hospital but cross site working will be required. OPAT run a clinic at Bournemouth Hospital once weekly.

Hours: OPAT have 30 hours available. Unfortunately we cannot offer any more than this. We work 7.5 hour shifts.

Main duties of the job

The UHD Hospital at home pathway is exciting and always evolving. Working through pathways in order to achieve flow and get patients back into their own environment. With the aim of delivering a high level of person centred acute care outside of a hospital ward environment. Safe practice is therefore essential.

Patients who meet defined eligibility criteria and have a ‘Criteria to Reside’ in acute hospital care, may be able to have observations, monitoring and treatments easily delivered at home, under the right supervision of our specialist teams.

You will be work alongside a team of band 7’s ECPS to run a daily clinic at the Poole hospital site. This clinic sees patients to assess their treatment plan, take bloods, deliver antibiotics and provide other treatments. Other clinical duties involve screening of patients who are appropriate for the service. You will work closely with our wider MDT including; the lead Consultant Microbiologists and Antimicrobial pharmacists as well as engaging with clinical teams from a variety of specialities and community teams including patients GPs.

The Hospital at Home service is continually developing and being part of this will be part of your role.

Your role will also be vital in assisting with the development and management of the service, service improvement and governance.
