Job overview

This Specialty Doctor post offers the opportunity to work as an integral member of the Bristol Care Home Interface Project, an interdisciplinary team providing proactive and holistic geriatric review to care home residents across BNSSG funded for 15 hours per week.

The post-holder will be part of a team comprised of physician associates, specialist clinical pharmacist, trainee Advanced Clinical Practitioner, GPwERs and geriatricians.  The team aim to improve NHS support in care homes and strengthen links between care homes, community services and primary/secondary care.

We are looking for a GP with a specialist interest in frailty, or experienced frailty doctor, who can work as an independent practitioner when seeing patients in care homes and supervise more junior multidisciplinary team members. Please refer to the post-holder responsibilities within the job description.

Main duties of the job

The post-holder responsibilities will include:

  • Provision of expert clinical care to residents of care homes involved in the project, including detailed medication review and advance care planning.
  • Liaison with next of kin for many residents to inform best interests decisions
  • Clinical assessment of residents with Parkinsons Disease and other Movement Disorders residing in care homes, and presentation of these patients at the Parkinsons MDT Hub meeting when needed
  • Clinical supervision for non-doctor medical professionals and doctors in training
  • Contribution to non-clinical tasks such as managing the team caseload
  • Contribution to governance and policy
  • Involvement in audit, research and service development, specifically data collection and analysis of project outputs
  • Onward planning for service development (including future funding applications) which supports the objectives of the broader organisation.
  • Engagement with key stakeholders to achieve cross agency relationship building

This Specialty Doctor post offers the opportunity to work as an integral member of the North Bristol Care Home Interface Project, an interdisciplinary team providing proactive and holistic geriatric review to care home residents across BNSSG. The team now cover a large geographical area, from Thornbury to Weston; clinical assessments take place face to face in care homes. We aim to improve NHS support in care homes and strengthen links between care homes, community services and primary/secondary care.

Experience

Essential

  • Broad experience in care of the elderly in a community or hospital setting
  • Experience and interest in teaching of staff, both undergraduate and postgraduate and from all disciplines
  • Evidence of clinical audit (quality assessment/process improvement)
  • Experience of influencing and persuading others to adapt or change, resulting in improvements to clinical practice/patient care

Desirable

  • Knowledge and skills in specialist areas relevant to the care of elderly patients, such as Movement Disorders, Palliative Care, Dementia
  • Experience of working in care homes
  • Relevant publication in scientific journals, presentation at local/national conferences

Qualifications & training

Essential

  • Full GMC registration
  • MRCGP, MRCP or equivalent
  • Completed at least four years’ full-time (or its equivalent gained on a part-time or flexible basis) practice as a doctor, with at least two years frailty experience
  • Evidence of Continuing Professional Development (CPD), including regular appraisal.
