We are looking for an enthusiastic, skilled and motivated Physiotherapist to join our established Stroke Early Supported Discharge (ESD) team, delivering care across Mid and North Hampshire.

You will play a key role in enabling people to leave hospital earlier and recover safely and effectively in their own homes, delivering high-quality, evidence-based stroke rehabilitation.

Main duties of the job

The Stroke Early Supported Discharge Team (ESD) facilitates earlier discharge from hospital for stroke survivors, ensuring individuals receive the appropriate intensity of rehabilitation following discharge home from hospital. The pathway consists of Occupational Therapists, Speech and Language Therapists, Physiotherapists, Specialist Stroke Nurse, clinical psychologist and Rehabilitation Assistants. There are close links to the Twyford ward at The Royal Hampshire County Hospital in Winchester, the Community ICT service, the local Stroke Association, Reablement services and local exercise providers.

The post holder will work within the Stroke ESD pathway providing Specialist Physiotherapy assessment and rehabilitation for patient's identified as being suitable for the service.

The majority of clinical work will take place within the patient's home or temporary residence. Access to a vehicle and current driving licence is essential for this role.

Detailed job description and main responsibilities

  • To be professionally and legally accountable for all aspects of own work within scope of professional practice.
  • To undertake a comprehensive assessment of patients including those with diverse or complex presentations / multi-pathologies, using investigative and analytical skills.
  • Formulate and deliver an individual therapy management and/or treatment plan based on a sound knowledge of evidence based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment sills. To evaluate patient’s progress, reassess and alter treatment programmes as required. Involve carers with treatment when appropriate, which can involve teaching and assessment of their ability to be involved.
  • Formulate accurate prognoses and recommend best course of intervention, developing comprehensive discharge plans, including appropriate referrals to other specialities.
  • Work within trust clinical guidelines and professional guidelines and to have a good working knowledge of national and local standards and monitor own and others quality of practice as appropriate.
  • To demonstrate a sound understanding of clinical governance and clinical risk and to demonstrate this in daily practice.
  • To be responsible for maintaining accurate, comprehensive and contemporaneous patient treatment records in line with professional standards of practice.
  • To be responsible for the safe and competent use of equipment, gym equipment, patient appliances and aids, adhering to the medical devices policy, and ensure that junior staff / assistants attain competency prior to use.
  • To comply with the Trust Manual Handling Policy and local therapeutic handling guidelines at all times with / without the use of equipment / machinery.
  • To be responsible for maintaining an accurate and up to date record of patient statistics
  • To demonstrate the physical ability to carry out treatments and interventions that require exertion of moderate physical effort on a daily basis. This frequently includes heavy patients and confined spaces.
  • To demonstrate the mental ability to retain concentration and remain focused throughout a working day.
  • To work as a lone practitioner in accordance with the lone worker SOP this may be in the form of extended hours, on call, community and off site and seven day working.
  • If appropriate, to be member of the respiratory on call rota as per the on call policy.

  • Car driver with valid driving license for UK use
  • Ability to travel between ACH (Alton), BNHH (Basingstoke) and RHCH (Winchester) sites as required at short notice.
  • Able to occasionally deal with distressing circumstances pertaining either to patients or colleagues
  • Expectation to participate in 7 day working patterns as required

Desirable

  • Able to recognise stress in others & act on it appropriately
  • Experience as part of an oncall respiratory rota

Skills and Abilities

Essential

  • Effective verbal and written communication & interpersonal skills.
  • Demonstrate or show potential to lead a team
  • Effective participant in multi-disciplinary team meetings
  • Demonstrate understanding and appropriate application of all principles surrounding risk management, data protection, consent, confidentiality etc

Desirable

  • Undertaking of Departmental Officer role e.g. Steward, H&S Rep, Work Experience co-ordinator, Assistant Mentor
  • Presentation skills
  • Audit experience

Knowledge and experience

Essential

  • Evidence recent and continuous appropriate band 5 rotational experience
  • Demonstrate effective management of own workload and autonomous practice
  • Evidence of undertaking relevant post graduate training/ CPD including reflective practice
  • • Knowledge & experience in a wide range of neurological conditions and awareness of evidence based practice

Desirable

  • IT Skills
  • Assist in audit/ research projects
  • Show evidence of doing or have potential to supervise and teach more junior staff, students and other members of the healthcare team
  • Evidence of researching HHFT as an employer and wanting to work for organisation

Training and Qualifications

Essential

  • HCPC registered physiotherapist
  • BSc (Hons) physiotherapy or equivalent
  • 2 plus years post qualification with wide range of basic rotations covered

Desirable

  • Normal movement/basic bobath course
  • movement science course or equivalent
  • MSc module or evidence of extended qualification
