Job summary

We are seeking a knowledgeable individual with highly developed interpersonal and communication skills and has a passion for delivering excellence in care, driving improvements and developments across the stroke service.

Successful applicants will need to demonstrate an interest and sound clinical knowledge as well as an understanding of the specialist care and management required by patients across the pathway which includes inpatient, outpatient, and an outreach service to support patients, carers and staff with specialist advice, education and training in Stroke and its associated complications. Main duties of the job

Ensure provision of expert specialist advice and knowledge to patients, carers, and staff regarding stroke management

Accountable for the planning and delivery of patient care in relation to stroke.

Pro-actively manage own workload and clinical caseload of patients, making decisions regarding care, treatment and support required.

Liaise with Stroke Physician Consultants, co-ordinate the delivery of the stroke patient pathways using effective evidence-based practice and national standards for patient safety and monitoring.

Manage/lead specialist nurse led clinics and playing a key role in optimising complex treatment regimes, supporting hospital discharge and preventing avoidable hospital admissions.

Managerial

Attend meetings regarding stroke care and represent STSFT as an organisation.

Participate in service meetings, stroke CG and directorate level discussions.

Contribute to MDT meetings ensuring patients are appropriately reviewed and treatment options discussed.

To represent the service and play a key role in ensuring that national and local standards/targets are met.

Audit / Research / Governance

Participate in audit, research and governance activity relevant to the stroke service.

Continuously improve clinical care standards in line with the STSFT CG framework

Exercise accountability as set out in the NMC/HCPC Code

Comply with the trusts vision, values and behavioural compact.

Job responsibilities

To coordinate the delivery and development of the specialist service through the use of effective evidence based practice in line with agreed national standards for patient safety and monitoring. To contribute to the organisation of multidisciplinary, patient centred care.

To be accountable for the planning and delivery of safe, effective and compassionate patient care in relation to dedicated pathways of care, working as an autonomous practitioner within the specialism.

To provide specialist clinical advice and support to patients and clinical staff within the Trust on the assessment, management and care of patients within the specialist area.

To continuously improve clinical care standards in line with the clinical governance framework.

To exercise accountability as set out in the NMC Code.

To ensure effective communication internally and externally regarding patient care and resources.

Provide education and training to clinical staff across the Trust. Provide expert specialist clinical advice and support to patients and clinical staff within the Trust on the management and care of patients within the specialist area.

To provide direct access for patients, either face to face or telephone consultation.

To represent the service nationwide.

To play a key role in ensuring that national and local standards/targets for specialist area are met.

To manage/lead practitioner led clinics in relation to reviewing patients and play a key role in optimising complex treatment regimes, facilitating hospital discharge and preventing avoidable hospital admissions.

To undertake research relevant to the needs of the specialist area.
