Job summary

This role is advertised as a Band 6, however we would welcome Band 5 applicants to apply who are interested in a Band 5 to 6 developmental role.
We are looking for an enthusiastic and forward-thinking Nurse or Physiotherapist with a passion for respiratory care to join our friendly multi-disciplinary team. You will be able to further develop your chronic respiratory disease management skills through supporting patients on a mixture of patient pathways, including delivering Pulmonary Rehabilitation, assessing patients on and for oxygen therapy, as well as supporting patients with acute COPD exacerbations at home through our Supported Discharge and Admission Avoidance pathways.

Main duties of the job
Work within the Southwest Hampshire locality, and when required provide cross cover to the mid and north Hampshire localities to maintain service delivery across the region.
Build strong working relationships with acute Trust partners, particularly respiratory consultants.
Plan, deliver and support education sessions for patients and professionals.
Assess, plan and deliver evidence based care for respiratory patients, following current best practice guidance.
Provide a range of interventions including self management support, medicines management, chest clearance, breathlessness management, oxygen assessment and prescribing, Pulmonary Rehabilitation, and end of life care.
Manage own clinical workload, delivering both individual and group interventions.
Contribute to service development, promoting multidisciplinary and evidence based practice.
Work with external services to provide holistic patient management.
Supervise junior staff and students.
Liaise with primary care, community teams and third sector partners to ensure seamless patient care.
Support the evaluation and procurement of medical equipment used by the service.
Work with respiratory related charities to promote awareness and understanding of respiratory conditions.
Contribute to the development and review of service policies and protocols.
Collect and present data to demonstrate service impact and inform future developments.
Support research, quality improvement and the annual audit programme.
