This post requires the post holder to have a PVG Scheme membership/record. Community Rehabilitation

Part – time (30 hours) Permanent

An exciting opportunity has arisen to join an established community Rehabilitation team consisting of both Physiotherapists and Occupational Therapists .

As part of the Home First Programme the team delivers an admission prevention service for patients and on going rehabilitation services to support the longer term management of patients either in their own home or community bases . The post will also support staff development within the Physiotherapy Service and work collaboratively with primary and community colleagues.

Candidates mustbe HCPC registered have previous Community physiotherapy experience and be dynamic in utilising a senior level of decision making, leadership and clinical judgement to ensure that the health needs of patients are met through the delivery of high quality assessment and treatment

As well as a clinical caseload, the postholder will have staff and student supervisory responsibility ..

There is an active in-service training programme and a strong commitment to professional development
