Job summary

We have an exciting opportunity for a Specialist Occupational Therapist at Buckinghamshire Healthcare NHS Trust to join the Rapid Response & Intermediate Care (RRIC) Teams across Buckingham and Marlow. In this rewarding community-based role, you will be responsible for delivering active case management programmes for patients with complex needs, ensuring person-centred interventions are tailored to meet individual requirements. You will work closely with Advanced Community Practitioners to support workload allocation across the team, coordinate care plans, and contribute to the delivery of high-quality, timely care. Working collaboratively with a range of healthcare professionals, you will demonstrate expertise in leadership, care planning and coordination, medicines management, and the delivery of effective care and treatment to support patients in the community.

Main duties of the job

As a Specialist Occupational Therapist, you will be responsible for proactively identifying individuals who are frequent users of community and primary healthcare services and those at risk of unplanned hospital admissions. You will support the management of referrals into the service, ensuring patients are appropriately triaged and directed to the most suitable interventions. You will utilise your specialist clinical expertise to evaluate functional needs, deliver targeted therapeutic support, and monitor patient progress. In addition, you will oversee a dedicated caseload of individuals living with long-term conditions, helping them maintain independence, reduce avoidable healthcare utilisation, and achieve improved health and wellbeing outcomes.

Job responsibilities

  • To develop, review, and implement clinical protocols, operational procedures, and service guidelines that support the effective delivery of community-based services and ensure adherence to best practice standards.
  • To oversee and monitor the quality of delegated activities, providing guidance, feedback, and professional oversight to colleagues while maintaining high standards of clinical governance.

  • To contribute to workforce development through the delivery of induction programmes, mentorship, clinical supervision, and educational support for Community Health Assistants, Practitioners, and other healthcare professionals.
  • To participate in service development initiatives, supporting the introduction of new ways of working, contributing to policy implementation, and promoting continuous improvement across the service.
  • To ensure compliance with organisational policies relating to confidentiality, information governance, equality and diversity, health and safety, and risk management, maintaining a safe environment for patients, visitors, and staff at all times.

Person specification

Qualifications

Essential

  • HCPC registered Occupational Therapist
  • Degree or Diploma in a relevant healthcare profession.
  • Minimum two years' post-registration NHS community or inpatient experience.
  • Experience in rehabilitation, intermediate care, or long-term condition management.
  • Full UK driving licence, own vehicle, and evidence of ongoing CPD. Evidence of ongoing CPD.
