As part of a close MDT, you will work together with your patients to identify meaningful goals, provide assessments, identify appropriate interventions and support their independence and discharge from hospital.
The post holder will carry an independent caseload of patients and deliver occupational therapy intervention to a high standard with the ethos of providing evidence based, high quality interventions.
The successful candidate will work closely with the MDT, Principal OT, Discharge Liaison Team and Local Authorities to ensure effective cross boundary and agency working.
To provide Specialist Occupational Therapy assessment, intervention and rehabilitation to patients to ensure improved functional recovery, prompt discharge and to enable the person to remain in their home.
You will provide supervision and support to new/less experienced members of the team and student Occupational Therapists and have a passion for delivering evidence based, quality intervention using research, audit and quality improvement projects.
The post holder will be flexible to service needs, possess excellent self-management skills and administer their caseload efficiently to ensure effective intervention focused on preventing avoidable admissions, rehabilitation and efficient discharge planning, depending on setting.

Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.
