We have a exciting opportunity for a Band 6 Occupational Therapist posts within the Ellesmere Port West Care Community Team based in Ellesmere Port.

You will be part of an Integrated Care Community Team and be responsible for the Therapy provision for our patients.

This role is working the above team, although the successful candidate will need to be flexible to support our colleagues within our 8 Care Community Teams in the Neighbourhood Group.

You could already be a Band 6 wishing for a change, or currently an experienced Band 5 who would like to take the next step in your career and practice in a community setting.

Main duties of the job

To work autonomously as a specialist therapy practitioner within an integrated Care Community Team to deliver a high-quality Occupational Therapy service through assessment and treatment of patients referred with complex rehabilitation needs

To work as a member of a multi-disciplinary team and communicate information to those agencies (either health or social background) involved, to meet the needs of the patient & plan effective care.

To participate in departmental In-service Training and Journal Club activities to further consolidate knowledge base and specialist clinical skills. Participate in the CPD programme to share these skills with therapy and nursing colleagues.

To provide supervision, teaching and support to senior and junior staff, assistant staff and act as a clinical educator for undergraduate students.
