
Job overview
The post holder will work as part of the Reablement Service comprising occupational therapy, physiotherapy, reablement support workers and other local authority staff. There are currently 4 Reablement Teams based in different venues across Calderdale. The teams are an integrated partnership between health and social care, and support and rehabilitate people in their own homes, either facilitating early discharge from hospital; preventing the need for admission to hospital, residential or respite care; reducing long-term dependency on care; and promoting self-management.
The Reablement service provides a time limited period of assessment, treatment, rehabilitation and reablement, according to patient centred goals for people aged 18 years and over. It comprises 4 geographically based teams and you may be expected to work across the 4 teams or cross site as required.
Service users may have a range of needs and conditions, such as deconditioning, frailty, neurological conditions such as stroke, acute and chronic musculoskeletal problems, a history or risk of falls, respiratory disorders, orthopaedic conditions including recent surgery, and complex multi pathology conditions which may include patients with early dementia.
The post holder will be an employee of Calderdale and Huddersfield NHS Trust and will be line managed as such. However, the post holder will work in an integrated service alongside employees of the local authority.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

- Occupational Therapy Degree/Diploma
- HCPC registration
- Clinical Educator Qualification
KNOWLEDGE, EXPERIENCE AND EXPERTISE

- Recent experience demonstrating ability to assess, treat and evaluate care in Rehabilitation and Long-Term Conditions
- Experience of managing rehabilitation and goal attainment within time-limited service,
- Knowledge of relevant legislation relating to hospital discharge, Intermediate Care, and local initiatives and drivers
- Specialist knowledge of Falls/ frailty/ complex care/dementia/aids and adaptations
- Comprehensive knowledge of appropriate and available assessments and approaches in the specialty.
- Proven ability in implementing Clinical Governance
