<p><strong>Make a Real Difference in Stroke Rehabilitation</strong></p><p>Every stroke survivor\'s recovery journey is unique. As a <strong>Specialist Community Physiotherapist</strong>, you will play a pivotal role in helping individuals regain independence, confidence, and quality of life following a stroke. This is an opportunity to combine expert clinical practice with leadership, working within a dynamic multidisciplinary team dedicated to delivering outstanding patient-centred care across Wiltshire.</p><p><strong>Base:</strong> Wilton Health Centre</p><p><strong>Hours:</strong> 28 hours per week </p><p><strong>About the Role</strong></p><p>We are seeking a highly motivated and experienced clinician to join our <strong>Early Supported Discharge (ESD) Service</strong>. In this role, you will provide specialist neurological assessment, treatment, support, and advice to individuals recovering from stroke, enabling them to achieve their rehabilitation goals within their home and community settings.</p><p>You will also provide professional and clinical leadership to registered practitioners and support workers, helping to develop a skilled and supportive workforce that delivers high-quality stroke rehabilitation services.</p><p><strong>Package Description</strong></p><p>As a Specialist Commun
