Job overview

An exciting opportunity has arisen for an enthusiastic and motivated pharmacist to develop specialist skills within our Infection & Immunity team, working across HIV, Sexual Health and Hepatitis C services. You will join a supportive multidisciplinary team and play a key role in delivering innovative, patient-centred care while contributing to the ongoing development of the service.

You will be responsible for:

  • Delivering patient-centred clinical pharmacy services in both inpatient and outpatient settings.
  • Providing clinical oversight of the HIV home delivery service.
  • Supporting the provision of high quality, patient centered pharmacy services to GUM/Sexual reproductive health and Hepatitis C
  • Leading and contributing to audit, quality improvement projects and service reviews.
  • Supporting the education and development of pharmacy staff.

The post holder will support the Highly Specialist Pharmacist in directorate work, audit and research. This role combines advanced clinical practice and leadership responsibilities, providing specialist advice to patients and the multidisciplinary team to optimise medicines use and support adherence

We are seeking a self-motivated and experienced clinical pharmacist with excellent organisational, communication, and interpersonal skills.

Main duties of the job

The post holder will support the Network Lead Pharmacist and Highly Specialist Pharmacist for Infection and Immunity in promoting high quality, cost-effective and safe prescribing. The post holder will provide clinical pharmacy support to the Infection and Immunity departments across the Trust. The post holder will participate in education and training of Pharmacy staff rotating in the Infection and Immunity pharmacy team at Bart’s Health, and will assist in the provision and development of clinical pharmacy services within the service

The post holder will be responsible for providing patient-focused pharmaceutical care, defined as the responsible provision of medication to achieve definite outcomes that improve patients’ quality of life. The post holder is to communicate effectively with patients and the wider MDT to optimise their pharmaceutical management through liaising with other healthcare professions to design, implement and monitor therapeutic plans to optimise clinical outcomes, minimise adverse reactions and improve medicines safety.
