JOB SUMMARY

The post holder will lead, develop and deliver enhanced medicines optimisation services to people with Diabetes in accordance with the Northern Ireland Diabetes Strategic Framework “Community Care Model for Diabetes”. The post holder will also lead, develop and deliver diabetes clinical pharmacy services for adult inpatients in Altnagelvin Area Hospital. This will include direct care for patients within the above work streams, provide medicines optimisation support and education to the multi professional inpatient and primary care teams and directly contribute to the overall targets and objectives of these work streams.

MAIN DUTIES & RESPONSIBILITIES

Clinical Service Delivery

1.1 Attend weekly multidisciplinary team meetings and contribute to and directly manage adult patients with diabetes with a particular emphasis on cardiometabolic co-morbidities.

1.2 Work as part of the inpatient diabetes service leading or participating in ward rounds managing the care of adult inpatients with diabetes.

1.3 Lead and develop an enhanced clinical pharmacy service to identified patients within the above work streams. This will include working within outpatient multidisciplinary clinics aligned to the Community care model for Diabetes.

1.4 Liaise with clinical pharmacists, physicians, specialist nurses, allied health professionals, GPs and community pharmacies regarding pharmaceutical interventions and recommendations.

1.5 Link with and develop pharmaceutical models which work closely with the voluntary sector and community pharmacy.

1.6 Involve service users in the development of clinical pharmacy services across the interface using personal and public involvement initiatives.

1.7 Be an active independent non-medical prescriber for patients within the above work streams. Prescribe in accordance with Trust policy, the Royal Pharmaceutical Society Prescribing Framework, relevant legislation and professional code of practice.

1.8 Communicate specialist pharmaceutical care needs across care settings for patients with complex and multiple pharmaceutical care needs.

1.9 Develop prescribing guidelines, protocols, patient group directions, procedures and clinical management plans for adult in patients with diabetes in collaboration with appropriate stakeholders.

1.10 Demonstrate the ability to provide safe, clinically effective and cost-effective use of medicines in accordance with Trust policy.

1.11 Ensure the security of medicinal products in all settings that the post-holder comes into contact with including wards, clinics and primary care.

1.12 Where appropriate, be an expert member of Network, Trust, and Division specific committees and national bodies & networks relating to the use of medicines with the aim of proving expert advice within the defined clinical area e.g. Drugs and Therapeutics Committees, Risk Management Committee, UKCPA.

1.13 Assist with the implementation of regional and national guidelines, managed entry of new drugs and the feedback to the Trust on compliance with these. This will include working regionally with SPPG to implement agreed medicines management initiatives linked with diabetes prescribing.

1.14. The post holder will undertake and publish practice research and enable team members to do likewise and identify areas for quality improvement and clinical audit.

1.15 Ensure that work practices and the working environment conform to the requirements of current legislation and professional recommendations.

2. Communication & Information Management

2.1 Communicate effectively with staff and maintain productive working relationships amongst your staff and with others.

2.2 Report and record all accidents, untoward incidents, sickness and absence, within your area of responsibility, using appropriate forms.

2.3 Deal with and report complaints, in keeping with Trust Policy, and assist with investigations, when necessary.

3. Quality

3.1 Ensure that the needs of patients and their carers are at the core of the way that the Trust delivers services.

3.2 Work within the Trust’s governance framework to ensure policies and procedures meet relevant statutory requirements, professional standards, policy guidance and workforce and regulatory requirements.

3.3 Support the Trust in advancing its compliance with Risk Management Controls Assurance Standards and to contribute to improving governance across the organisation.

3.4 Promote a culture of continuous service improvement amongst your staff, encouraging their participation and that of service users in reviewing and modernising current services and in service development

3.5 Take a lead role in the Trust Insulin Safety Group. Contribute to the investigation of all levels of medicine related incidents and agreement of learning action plans to help make promote safer care of people with diabetes in hospital.

4. Financial & Resource Management

4.1 Develop and monitor appropriate controls within scope of responsibility in accordance with the Trust’s Schedule of Delegated Authority

4.2 Adhere to agreed policies and procedures including standing financial instructions.

4.3 Ensure that the Trust’s funds are effectively used, that financial commitments represent value for money and comply with standing financial instructions and Trust protocol.

4.6 This post holds no budgetary responsibility but contributes to the control of cost and expenditure within the scope of the role.

4.7 Support service developments through contributions to business cases and negotiation with stakeholders outside pharmacy.

5. People Management

5.1 Establish a supportive, fair and open culture that encourages and enables staff, within your area of responsibility, to meet required performance standards.

5.2 Identify, at least annually through performance appraisal, training and development needs of staff in line with the NHS Knowledge and Skills framework and to carry out the development review process as required.

5.3 Participate in the recruitment and selection of new staff as required.

5.4 Participate in the orientation and induction of new staff and in the development of good interpersonal relationships.

5.5 Encourage good practice and act as a role model, give leadership and support to staff and encourage effective teamwork within your area of responsibility.

5.6 Manage and document grievance and disciplinary matters in keeping with Trust policies.

5.7 Take responsibility for managing and improving attendance of staff in accordance with the Trust’s Managing Attendance Protocol.

5.8 Establish and promote a supportive, fair and open culture that encourages and enables all parts of the team to have clearly aligned goals and objectives, to meet the required performance standards and to achieve continuous improvement in the services they deliver.

5.9 Ensure access to skills and personal development through appropriate training and support.

5.10 Promote a culture of openness and honesty to enable shared learning.

5.11 Encourage and empower others in their team to achieve their goals and reach their full potential through regular supportive conversation and shared decision making.

5.12 Adhere to and promote Trust policy and procedure in all staffing matters, participating as appropriate in a way which underpins Trust values.

6. General Management Responsibilities

6.1 The post holder will promote and support effective team working, fostering a culture of openness and transparency. The post holder will ensure that they take all concerns raised with them seriously and act in accordance with ‘Your right to raise a concern (Whistleblowing)’ policy and their professional code of conduct, where applicable.

6.2 The post holder will, in the event of a concern being raised with them, ensure it is managed correctly under the ‘Your right to raise a concern (Whistleblowing)’ policy, and ensure that feedback/learning is communicated at individual, team and organisational level regarding the concerns raised, and how they were resolved.

7. Strategic Planning

7.1 Contribute to strategic and operational committees related to the planning, development and delivery of multidisciplinary diabetes services as appropriate.

7.2 Responsible for the continual review of pharmaceutical service provision to people with diabetes within the trust. Where necessary plan and implement changes to ensure that future service development needs are met.

7.3 Ensure that the output and quality of the service is maintained in relation to short and longer term objectives.

8.0 Education and Training

9.1 Where appropriate act as an Education Supervisor for foundation year and junior pharmacists undertaking post graduate training.

9.2 Where appropriate support the teaching of these foundation year and junior pharmacists within the specialised clinical area.

9.3 Contribute to delivery of Foundation training year and pharmacist non-medical prescribing programmes by acting as a Designated Prescribing Practitioner (DPP) as appropriate.

9.4 Provide diabetes education and training to medical, nursing, pharmacy and allied professional staff and students.

9.5 Contribute to the training of undergraduate pharmacy students on placement in the Trust.

10. Pharmacy

10.1 Undertake all mandatory training required for the role.

10.2 Maintain professional competence through engaging in at least the minimum amount of continuing professional education required by the Pharmaceutical Society of Northern Ireland.

10.3 Maintain competencies within general dispensary and clinical areas to fulfil requirements of the role.

10.4 Participate in extended hours, weekend working and bank holiday rotas in accordance with local requirements.

10.5 Participate in the out-of-hours emergency on-call rota.
