Job summary

CPG Rehab, Re-enablement and Recovery unit is a bed-based facility supporting people to recover and regain their independence following a period of illness, trauma, surgery, or significant event. It supports hospital discharges, prevents admission to hospital or long-term care and promotes independent living. Its a multi-disciplinary service with Nurses, Physiotherapists, Occupational Therapist, Social Workers, and Care Staff.

Main duties of the job

Hours: 37.5 a week, permanent, pay award pending

Salary Range 5 £29,317 - £35,715 per annum

As a social worker within the team at CIU you would work:

As part of a wider multi-disciplinary team to coordinate and support admissions and discharges within the unit.

To assess care needs and to implement appropriate care package/ placement within the Care Act Legislation.

To act as a role model in the adherence of the Mental Capacity Act.
