Main duties of the job

As a Social Worker you will be:
- Referrals to LADO and Local Authority in line with safeguarding and assessments
- Educating Hospital staff about relevant safeguarding matters.
- Completing a full annual social care reassessment within the CPA format
- Facilitating access to welfare rights, housing, advocacy services and associated support services
- Participating in 1:1 supervision
To be successful in this role, you'll need:
- A Social Work degree
- Registered with Social Work England or applied for
- Experience within similar environment
- Experience with working with vulnerable adults
Where you will be working:
Location: Penperlleni, Nr Pontypool, Monmouthshire, Wales NP4 0AH
You will be working at Aderyn, a high quality step down service for men with enduring mental health conditions sets a sector-leading example of clear care pathways for service user with a focus on recovery. As well as a 17 bed rehabilitation unit there is also a 2 bed open rehabilitation cottage.
The unit is situated in a picturesque part of Wales with good transport links. You will working alongside a multidisciplinary team to provide service users with opportunities to improve their independence and prepare for an independent life in the community. Individual and collaborative therapeutic timetables are developed for each patient and individual psychological therapies are offered.
