As a Children and Young persons (CYP) social worker working across the inpatient and day service provision, you will play a crucial role across health & social care systems by providing social work intervention, advice and support to patients, carers and families with a variety of complex mental health need. The post holder will form part of a social work structure across CYPS, working within and alongside a multi-disciplinary team.  The post holder will deliver evidenced based Social Work interventions, ensuring a holistic approach to patient care by promoting positive social and environmental outcomes

Main duties of the job

This role requires effective communication and strong partnership working across health, social care, voluntary and statutory services to improve outcomes for children and young people with complex mental health needs. The post holder must communicate sensitive information clearly and compassionately to service users, families and carers, overcome barriers to engagement, advocate for service users' needs, and contribute to multidisciplinary meetings, risk discussions and care planning. Professional relationships should be built on respect, collaboration and therapeutic engagement, while adhering to confidentiality and information governance standards. The role requires a recognised Social Work qualification, Social Work England registration, and knowledge of key legislation including the Mental Capacity Act, Care Act, Mental Health Act, Human Rights Act and safeguarding procedures. The post holder should demonstrate sound clinical reasoning, reflective practice, risk assessment skills and the ability to assess social care needs.

Detailed job description and main responsibilities

Working within a multidisciplinary team, responsibilities include completing assessments, developing and reviewing care plans, supporting discharge planning, managing risk, securing appropriate care funding and maintaining accurate records. The post holder must organise and prioritise their workload, work across a variety of settings, and contribute to service improvement, audits and evidence-based practice. They are expected to maintain professional development, participate in supervision, support staff and students, use resources responsibly, and practise autonomously within professional standards while managing a defined caseload with appropriate support.
