Job summary

An exciting opportunity has arisen for a friendly and supportive Social Prescriber to join our team in Gosport.

In this role, you will work with individuals and groups utilising untapped community resources, support the facilitation of self- help groups and other innovative approaches to enable people to better manage their own health and wellbeing. You will aim to provide a service which helps members of the community access support to better self-manage their health and wellbeing and long-term health conditions.

You will be working within a team supporting a population of 85,000 patients in Gosport that are registered with Bridgemary Medical Centre, Blossom Health, Rowner Medical Centre, Solent View Medical Practice and The Willow Group, GP Surgeries.

Main duties of the job

Social Prescribers work 1:1 with people in the community, focussing on what matters to them and building a trusting relationship with the person referred to them. They create a shared personalised care and support plan and connect them to local community groups, voluntary and community sector organisations and services. The role helps people to work on their wider health and wellbeing, addressing health outcomes and wider determinants of health, such as debt, poor housing, and physical inactivity, as well as other lifestyle issues and low-level mental health concerns; by increasing people's active involvement with their local communities. This approach particularly helps people with long term conditions (including support for mental health), people who are lonely or isolated, or who have complex social needs which affect their wellbeing
