Job summary
Sudbury & West Suffolk Rural (S&WSR) Primary Care Network hasanexcitingopportunity for a social prescriber to join our practice teamon afull-time basis for 16 hours per week.
Social prescribers are responsible for providing non-clinical support by linking patients to community and voluntary services across their area, adopting a holistic approach to supporting patients to improve their health and wellbeing.We are looking for someone to join our team in order to provide this service across S&WSR PCN at, Clare Guildhall, and Siam Sudbury Surgeries.
Main duties of the job
The successful candidate will be responsible for providing one-to-one support, identifying issues that impact on patients' health and wellbeing, developing personalised care and supporting plans to address their needs, making onward referrals to community and voluntary sector services, and recording patient outcomes.

We are a regulated service under The Health and Social Care Act 2014 and therefore fall within the proposed regulation changes of 9 November 2021 which require staff who have face to face contact with patients to have completed a course of an approved COVID 19 vaccine, subject to limited exceptions, by no later than the 1st April 2022 .
