Job overview

We are developing adult mental health rehabilitation services in Cornwall in line with the NHSE transformation programme. Our provision includes a 12‑bedded inpatient unit and a growing Community Mental Health Rehabilitation Team supporting smooth transitions and helping people remain well at home.

Our caseload focuses on people with complex psychosis who may need short inpatient admissions for symptom stabilisation, medication review, physical health monitoring and support with daily living. We have created a dedicated transition pathway and are offering an exciting opportunity for a motivated, compassionate RMN to lead this work and support our Social Inclusion Workers.

You will take a person‑centred approach, using the Dialogue+ model to co‑produce individualised plans. A key part of the role is enabling timely, sustainable discharges from acute wards. Strong communication skills are essential, as you will work closely with families, carers, acute services, crisis teams, HTT, housing providers, VCSE partners and patient‑flow teams. You will also provide clinical supervision to Social Inclusion Workers, each holding a short‑term caseload.

Join us at a pivotal time as we expand our countywide service. Our team is compassionate, innovative and committed to high‑quality care. If you are interested—whether already in CFT or considering relocation—please get in touch for an informal conversation.

They  will provide clear clinical leadership and direction in the on-going benchmarking and good practice service evolution.
