JOB SUMMARY
The Directorate of Adult Community Services has established a Reablement service for older people. The purpose of this service will be to enable older people to regain the skills and confidence they may have lost as a result of poor health or as a consequence of having spent a period of time in hospital or residential care. It is fundamentally different from a standard domiciliary care service in that the Reablement support worker will not just meet the day to day needs of the older person but actively help them to achieve maximum independence. The focus will therefore be on helping older people “to do for themselves” in their own home rather than “doing to or on” them. It is envisaged that those assessed as being suitable for a period of Reablement will have had their maximum independence restored within 6 weeks. The Reablement service will be provided over a seven day week.
The reablement support worker is a key member of the Trust’s Reablement Team and will work under the guidance of Occupational Therapists.
KEY DUTIES / RESPONSIBILITIES
Actively encourage and work with older people to restore the confidence and skills they require for independent living in their own home. This will include working with the older person across a broad range of Reablement activities as per the individual’s care plan, e.g. looking after own personal care, meal provision, mobility, the use and care of their personal aids and appliances, developing and maintaining social networks and promoting general health and wellbeing.
Ensure all documentation in care plans is fully and accurately maintained to agreed standards and procedures.
Liaise with carers and family members to promote the Reablement ethos and secure their assistance with planned programmes/activities to restore the independence of their family member.
Report any changes in the older person’s circumstances or condition in accordance with Trust procedures.
Immediately report any “no access” visits that may occur.
Work as a member of the Trust’s Reablement service providing a seven day Reablement service delivering programmes to clients in their own homes.
Work collaboratively with team members of other Trust services to ensure the effective and timely exchange of information.
Attend and contribute to patient/client review meetings as required.
Treat all patients / clients and their carers with respect, dignity and courtesy and regard all matters relating to the individual and his / her circumstances as strictly confidential.
Summon appropriate assistance in the case of an emergency in accordance with Trust procedures

NVQ/QCF level 2 in a care related subject or 4 GCSES at A-C including English language or equivalent higher qualification and 1 years’ experience in a Health/Social Care related role.
Where NVQ 3/QCF 3 in a Care related subject is not already held, the successful applicant must be willing to undertake this training on appointment and complete this qualification within one year of commencement.
