The post holder is required to assist in the provision of safe, effective, compassionate person centred care.
The post holder will assist and support service users to maintain their independence in all aspects of the activities of daily living, including personal care and eating and drinking.
The post holder will work as a member of a team in the delivery of a wide range of therapeutic and recreational activities to meet the assessed needs of each individual Service User.
Working closely with the Centre staff and other professionals, the emphasis will be on promoting and maintaining independence, facilitating social inclusion and improving quality of life of older adults including those with a dementia illness.
Following a period of induction training, the major duties will include.
Participate in on-going team approach to Person Centred assessment of Service User needs.
Contribute to individual care plans based on a person centred approach and ensure the plan is monitored and reviewed in accordance with Trust procedures.
Implement, without direct supervision, individual group programmes in and outside the Centre.
Assist the Key Worker/ Senior Day Care Worker and Allied Health Professionals with the delivery of Programmes in accordance with Trust Policies.
Support Service users, by helping where necessary, with toileting and feeding to ensure identified physical and personal care needs are met.
Contribute to the moving and handling of Service Users to maximize their physical comfort in accordance with the Trust’s Safer Patient Handling Policy and individual risk assessments/safe systems.
Assist with the serving of meals, which may include helping to feed Service Users. Ensure compliance with individual risk assessments/safe systems and Centre risk log.
Observe and report on the progress/general demeanour of Service Users and maintain routine records as appropriate, in accordance with the Trust’s Client Access Policy.
Undertake work with individuals with complex needs and or displaying behaviour, which may presents as repetitive, challenging and demanding.
Communicate appropriately with Service Users.
Be conversant with first aid and undertake First Aid Training as required.
Assist as required with the administration and recording of medicines in accordance with Trust policies and guidelines.
Assist to maintain and promote the rights and dignity of older people and older people with a disability/ dementia at all times.
Assist to maintain and develop Service User involvement.
Liaise with carers as appropriate.
Assist with the checking of supplies in the Centre.
Adhere to control of infection protocols, general cleaning, tidying of equipment and materials after use.
Undertake Guide Help Duties if required.
Participate in a formal programme of induction, supervision, and staff meetings.
Participate in staff training and quality initiatives
Identify and report risk to Service Users and staff as appropriate.
Identify and report concerns regarding adult protection issues for further investigation.
Adhere to Day Care standards relevant to delivery of care and support to Service Users.
Ensure the Trust’s policy on equality of opportunity is promoted through his/her own actions and those of any staff for whom he/she has responsibility.
Co-operate fully with the implementation of the Trust's Health and Safety arrangements, reporting any accidents/incidents/equipment defects to his/her manager, and maintaining a clean, uncluttered and safe environment for patients/clients, members of the public and staff.
