Job overview

The Service Improvement function has evolved from supporting the ACD Programme and NCN arrangements to supporting the implementation of Integrated Neighbourhood Teams (INTs) through the PBC Programme. The role retains a strong focus on continuous improvement, stakeholder engagement, programme delivery and service transformation, whilst supporting the delivery of CbD principles across localities.

The post holder will work collaboratively across locality partnerships and INTs to understand local population health needs, identify opportunities for improvement and support the development of integrated models of care. The role adopts a neighbourhood-focused approach, ensuring that local intelligence informs service redesign, workforce planning and investment decisions.

Main duties of the job

The role works in partnership with stakeholders to facilitate system-wide transformation and demonstrate measurable improvements in population health, patient experience, workforce sustainability and service performance. The post holder will support the development of outcome frameworks, benefits realisation plans and evaluation methodologies that evidence the impact of INTs and wider PBC interventions implemented locally.

The role supports the development of business cases, implementation plans and programme governance arrangements and will provide regular reports and updates to divisional, locality and programme management structures. The post holder will provide assurance regarding programme delivery, funding utilisation, benefits realisation and achievement of agreed objectives.

THIS POST IS FIXED TERM/SECONDMENT FOR 6 MONTHS.
