Job summary
We are looking for an experienced and motivated Band 6 Physiotherapist to join our Fareham and Gosport Community Therapies Team.
Working as part of a supportive multidisciplinary team, you will provide high-quality Occupational Therapy assessment and intervention to adults within the community, supporting people to maximise their independence and achieve their rehabilitation goals.
You will manage your own caseload, using sound clinical reasoning to assess functional needs, identify and manage risks and develop individualised treatment and rehabilitation plans. You will work closely with patients, families, carers and colleagues across health and social care to support safe, coordinated care within the community.
You will contribute to the development of junior staff and support workers through supervision, delegation and sharing your clinical knowledge and experience.
We are looking for someone who is motivated, organised and confident working independently as well as part of a wider team. You will receive regular supervision and have access to learning, development and CPD opportunities.
HCPC registration as an Physiotherapist is essential. Due to the community nature of the role, access to a vehicle for work is required.
Main duties of the job
Undertake Occupational Therapy / Physiotherapy assessments of patients with diverse and complex conditions, providing a clinical diagnosis (where appropriate), development, and delivery of individualised treatment programmes.
To support the case management process by providing expert clinical advice and therapy to patients with a variety of complex conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission to secondary care.
To ensure that up to date written and electronic records, reports and activity data are maintained in accordance with professional and Trust standards.
To provide intervention in a variety of environments, e.g. clients home, hospital / healthcare settings, schools, work environment to meet client needs.
To provide clinical Occupational Therapy / Physiotherapy skills within the Community Therapy Team / Community Hospital Team(s), and all other provider organisations.
To ensure that delegated tasks and activity has been handed over clearly and are within a person's level of skill and competency.
To be skilled in assessing and treating a range of complex factors relating to the patient's condition, diagnosis and prognosis.
To provide specialist intervention tailored to meet the uniqueness of the individual.
i.e. complex manual handling and 24-hour posture management.
