We are pleased to be able to offer a Band 6 Physiotherapist role working in the West Berkshire area. We are a community team covering from Lambourn to Tilehurst.
We are an integrated team including Occupational Therapists, Physiotherapists, Therapy assistants, and admin support. The service supports multiple pathways including Community Reablement, Falls and Community Physiotherapy. This is an exciting time to join BHFT, as we develop services to work in an integrated way within the community in West Berkshire and across Berkshire West.
The successful candidate will be required to provide/ willing to develop skills in providing high quality, responsive services working as an integral member of the team. You will also work closely with social services and voluntary organization for adults with a wide range of physical impairments and mental health conditions.
The successful candidate will work autonomously under the guidance of senior clinicians (as required) having clinical responsibility for a designated caseload.
Main duties of the job
- Support hospital discharge referrals to promote the independence of service users on the caseload and identify any long-term needs. • Support falls pathway completing environment and clinical assessments to reduce risk of service users from falls within their homes • Provision of equipment to promote independence and safety. • Hold a caseload and work autonomously • Work flexibly to ensure urgent referrals are adequately managed • Supporting colleagues in the delivery of compassionate, safe and structured care • Plan and provide treatment in collaboration with others as appropriate • Provide guidance and appropriate delegation to therapy assistants in the team

The “must haves” for you to be considered for this role: 1. HCPC registration as a Physiotherapist and excellent communications skills 2. Understanding of intermediate care with experience of rehabilitation and hospital discharge 3. Car driver with a valid UK driving license with ability and readiness to be flexible within working days.

Essential
- Ability to demonstrate relevant experience as a senior physiotherapist in a community setting.
- Post-graduate experience covering core rotations
- Post-graduate experience of working as part of a physiotherapy team and multidisciplinary team
- Postgraduate / undergraduate experience in NHS
- Supervision of juniors / assistants / students
- Experience of delegating tasks
Desirable
- Postgraduate / undergraduate experience in ward-based rehabilitation
- Postgraduate / undergraduate experience in respiratory care
Additional Requirements
Essential
- Criminal Records Bureau clearance
- Ability to travel between multiple sites efficiently
- Full and Clean UK Manual Driving Licence
