Our Pharmacy Department are looking for an experienced hospital pharmacist, seeking new challenges in a clinically-orientated pharmacy service.

Main duties of the job

This will be a senior post responsible for leading on Preoperative care, Surgical care and project work

Ideally you will be an independent prescriber or be prepared to undergo the relevant training.  This is a unique opportunity to be part of a specialist pharmacy team working closely with multi-disciplinary clinical teams including consultants, registrars, specialist nurse practitioners, dieticians and psychologists.

Detailed job description and main responsibilities

  • Provide analysis of drug and patient information, including researching information in areas where information is not easily available.
  • Provide managerial planning and organisation to all aspects of the pharmaceutical service to the surgical admissions unit to ensure a timely and efficient service.
  • Write relevant protocols, procedures and guidelines for use within the surgical admissions unit and the wider Trust where medicines optimisation activities required.
  • Continual review of the service is expected through regular audit and development of medicines optimisation activities through project management and proof of concept pilots to establish new and innovative pharmacy services.
  • Act as the line manager for the junior pharmacists, trainee pharmacists and technicians providing clinical services in the surgical areas of the hospital.
  • Advise medical and nursing staff on complex drug use, and encourage compliance with the Trust formulary or clinical guidelines at all times, in order to foster rational and economic prescribing.
  • Be accountable for compliance with medicines legislation for safe and secure storage of drugs, including Controlled Drugs, in area of clinical responsibility.
  • Provide professional guidance, and ensure conformance with relevant legislation.
  • Participate in the Drugs and Therapeutics (D&T) Committee, by ensuring the preparation of papers to be considered by the D&T committee, within own specialist area.
  • Maintain appropriate records including medicines information enquiries, medication histories, pharmaceutical care plans, patient profiles and clinical interventions.
  • Plan and co-ordinate services to the allocated ward to ensure that they are delivered by Pharmacy in accordance with clinical governance criteria in relation to aspects such as clinical risk.
  • Advise the Chief Pharmacist on any service improvements suggested by local need or national initiatives that may impact on clinical pharmacy activities.
  • Cover the duties of other pharmacists during periods of sickness and absence and to undertake any other reasonable duties that may be required to ensure an efficient pharmacy services
  • Be responsible for input and use of patient and financial data on trust IT and paper based systems and generating reports from these systems.
