Job overview
An exciting opportunity has arisen for a Band 7 Senior Nurse Practitioner to join the newly commissioned Unscheduled Care Coordination Hub (UCCH) at Wirral University Teaching Hospital. This innovative role offers the opportunity to lead and influence a new model of urgent care coordination, improving access to community-based services and reducing unnecessary hospital admissions.
The UCCH centralises the management of Urgent Community Response referrals, including Category 3, 4 and 5 referrals from Northwest Ambulance Service. Working closely with community teams across Cheshire and Warrington, NWAS and system partners, you will provide clinical leadership, triage and coordination to ensure timely patient care.
Main duties of the job
To lead within the Unscheduled Care Co-ordination Hub, aligning with the National Framework for Urgent Care Response (UCR) 2 hr response.
To provide clinical assessment of acutely unwell patients and to support the frailty initiative by providing senior clinical support to both UCR and community staff.
To use established pathways and processes to provide a wide range of acute advise on treatment options in line with community nursing and social care assessments to patients who may potentially be at risk of a hospital admission.
To be an autonomous practitioner who has the experience and clinical confidence to assess the needs of the patient and determine if the patient needs further face to face assessment or whether they can be discharged at first point of contact.
To support the telephone triage of patients, working with both healthcare professionals and patients in determining the correct clinical pathway depending on their clinical presentation.
To provide support to junior members of the team in developing confidence around the decisions to use alternative to admission pathways.
