Powys Teaching Health Board is seeking to recruit two motivated and compassionate Senior Mental Health Practitioners (Band 6) to join our Local Primary Mental Health Support Service (LPMHSS).
LPMHSS in Powys provides timely assessment and intervention for individuals experiencing mild to moderate mental health difficulties, alongside support for those with stable severe mental health needs.
These roles offer flexibility to be based in Newtown, Llandrindod Wells, or Brecon/Bronllys, supporting communities across Powys.
As a Senior Mental Health Practitioner, you will play a key role in delivering the core functions of LPMHSS, supporting individuals through assessment, brief intervention, and onward referral where appropriate. You will deliver care using a range of modalities including face-to-face, telephone, and digital/online approaches.

Main duties of the job
- Undertake comprehensive mental health assessments of individuals referred to the Local Primary Mental Health Support Service (LPMHSS), including risk assessment, formulation, and care planning.
- Deliver brief, outcome-focused and evidence-based psychological interventions, in line with Matrix Cymru and NICE guidance, for individuals experiencing mild to moderate and stable severe mental health difficulties.
- Contribute to the delivery of the five core functions of LPMHSS under the Mental Health (Wales) Measure, including assessment, intervention, signposting, consultation, and onward referral where appropriate.
- Work collaboratively within a multi-disciplinary team (MDT), contributing to shared decision-making, case discussions, and service delivery.
- Support individuals in their recovery journey, promoting self-management, resilience, and access to appropriate services and community resources.
- Facilitate appropriate referrals and transitions between primary and secondary care services, ensuring timely and safe access to care pathways.
- Contribute to clinical governance, audit, service evaluation, and quality improvement activities, supporting the ongoing development of the LPMHSS.
- Provide informal mentorship, peer support, and leadership within the team.
