Senior Mental Health Liaison Practitioner

Job summary

We are looking for innovative, motivated and compassionate Advanced Clinical Practitioner to join the Mental Health Liaison Team within Queen Alexandra Hospital, Portsmouth. The service operates 24hrs a day, 7 days a week.

We offer a dynamic role to work with patients over the age of 18 with either an acute mental health concern or worsening of a pre-existing condition. Your role will include:

• Conducting full, holistic, biopsychosocial mental health assessments.
• Administering personalised interventions.
• Completing recovery focused care plans with patients.
• Providing specialist mental health assessments for working age adult patients and older people within Queen Alexandra Hospital.
• Advising non-psychiatry staff on outcome of assessment and formulate a care plan based on clinical intervention and risk management.
• Providing time limited liaison reviews when needed for those patients with more complex and enduring mental health needs whilst inpatient in Queen Alexandra hospital.
• Working positively and collaboratively as an integral part of the Adult Mental Health services, Older Peoples Mental Health services and effectively liaise with multi-agency colleagues, patients and their supporters.
• Providing mental health education, advice and training to clinical staff and students at Queen Alexandra Hospital.

Main duties of the job

The primary function of this role is to work as a one of a small team within our service of highly experienced mental health practitioners, this role will focus on triage and assessment of patients accessing the Emergency Department for emergency mental health care. Our team also provide assessment and mental health care to patients across the entire hospital site; at times you may have to work outside of the Emergency Department within the main hospital, dependent on the needs of the service/team.

The post-holder will be responsible for ensuring all those who present to the Emergency Department or are admitted to Acute Medical Unit with an acute mental health need, receive timely and appropriate care, in the right place, at the right time, by the right person. We are commissioned to provide a one hour response to patients in the Emergency Department, with a full biopsychosocial assessment to be undertaken and onward plan created within 4 hours. An expansive knowledge of primary care, secondary mental health services, and third-sector organisations will therefore be paramount.

Part of this role includes working as part of the leadership team within the Mental Health Liaison Team. You will be expected to undertake a variety of leadership duties including, but not limited to, supervision of staff, providing education and training to colleagues within the acute hospital, undertaking HR related activities pertaining to staff including performance management and sickness monitoring.
