Senior Health Care Support Worker (General Practice)

Job overview

This post is fixed term/secondment for 18 months to meet the needs of the service.   If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.

To assist the GPs and practice nurse team in the service and delivery of the care management of the practice population.

Responsible to: Clinical Co-ordinator, Operations Manager and clinical Key Teams agenda for Change (AFC) banding: 3

The post holder works under indirect supervision of the Clinical team leader and undertakes task and duties delegated by the practice nurse team leader or a suitably qualified regulated professional. They will work collaboratively with the general practice team to meet the needs of patients, following policy and procedures
