It is an exciting time to join the Haemoglobinopathy team at Sandwell and West Birmingham NHS Trust as we look forward to a new working model of integrating community and hospital Sickle Cell and Thalassaemia Service.

The team are looking for a friendly and dynamic nurse to join the team in a liaison nurse role, an integral part in facilitating the transition between paediatric services, but increasingly bridging the gap between hospital and home too.

We are looking for someone with experience of caring for people with haemoglobinopathy conditions or awareness of the challenges they face. You will need to have excellent communication skills, and be competent in venepuncture, cannulation and the use of CVADs. A prescribing qualification and clinical assessment qualification are essential. We look forward to working with you and helping you to fulfil your potential both professionally and academically.

Main duties of the job

All aspects of care of the haemoglobinopathy patient: screening and counselling on diagnosis.

Clinically supporting patients with their well being in an outpatient setting but also supporting during inpatient stays, delivering day treatments and managing the unwell patient.

Coordinating and participating in regional MDTs, teaching and raising awareness throughout hospital and community.

This role involves visiting patients in their own homes to deliver a holistic approach to care. As it requires working within the community, a full UK driving licence and access to a personal vehicle are essential.

Supporting the Lead nurse with service development.

Detailed job description and main responsibilities

The post holder will be a high-profile, dynamic nurse with a keen attention to detail and a holistic approach to care. As an accessible and responsive leader, they will hold 24-hour responsibility for patient care, staff coordination, service delivery, and resource management within a defined area. They will make informed decisions within their professional scope, involving patients and the public to meet agreed clinical standards and targets. The role includes managing and coordinating staff and clinical activities within the department’s liaison team, playing a key role in the multidisciplinary team, and leading change management initiatives within the haemoglobinopathy service. Strong communication links with divisional operational management and other stakeholders will be essential to ensure staff are effectively briefed and supported.

The post holder will provide specialist support and advice to healthcare professionals locally and regionally, working closely with patients to reduce hospital length of stay, promote independence, and encourage adherence to treatment plans and appointment attendance. They will lead a team of liaison sisters who support patients in the community, conducting home visits to assess individual needs and tailor support accordingly. Much of the work will focus on hospital avoidance, promoting compliance, and improving overall patient outcomes.

A full UK driving licence and daily access to a personal vehicle are required, as the role involves occasional lone working during home visits, necessitating vigilance and thorough risk assessment. The post holder will be required to prescribe medications to patients therefore a having the non-medical prescriber qualification is essential.
