You will become an integral part of a skilled and supportive multidisciplinary team comprising a Nurse Consultant, Service Manager, Clinical Nurse Specialist and Community Mental Health and Learning disability Nurses, supported by an experienced administrator.
The post holder will undertake comprehensive mental health and cognitive assessments for people living in care homes who present with suspected dementia, contributing to diagnostic formulation, care planning and ongoing management. They will provide specialist advice and support to social care staff, families and carers regarding person-centred care, support planning, and the non-pharmacological management of behavioural and psychological symptoms of dementia.
The role includes medication review and management, monitoring treatment effectiveness and tolerability. The post holder will work collaboratively with the wider multidisciplinary team, care home staff, families and carers to promote positive outcomes and high-quality dementia care.
