We are looking to recruit additional Senior Clinical Practitioners to work at our Oldham Urgent Care Centre (UCC) based in Oldham Town Centre and Oldham Primary Care Streaming Service (PCSS) based at Royal Oldham Hospotal. Full time, part time and bank positions available, Monday to Sunday between 08:00 and 00:00.

Main duties of the job

As a Senior Clinical Practitioner working within the Oldham locality you will holistically assess and clinically examine patients within your scope of competence. You will agree an effective management plan including prescribing where appropriate . In addition, you will have exposure to a multitude of streaming pathways and a broad Directory of Services (DoS) along with:

• Direct access to a wide range of community-based services.
• Direct referral rights to secondary care.
• Access to technology to support telephone and video consultations (full induction and training provided).
• Facilities to see patients face to face

Patients are referred into the Oldham Urgent Care service from several different routes. These include 111, NWAS, Oldham Care Homes, Health Care Professionals such as District Nurses. The service also manages a virtual ward and a covid oxymetry at home service.

Our Primary Care Streaming Service within Royal Oldham Hospital allows the assessment of low-acuity patients presenting to the Emergency Department, alleviating pressure within the department whilst providing high-quality assessment and treatment of patients.

Job responsibilities

• Holistically assess and clinically examine patients who fall within your scope of competence, through history taking, physical examination and appropriate use of tools to confirm diagnosis.
• Plan, implement and evaluate an effective management plan for the patient with support from senior colleagues.
• If you are a non medical prescriber (NMP) you will provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
• You will work towards your NMP qualification if you are not currently a NMP.
• Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations.
• Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

• Embrace the use of digital technology to support safe and effective patient care.
• Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
• Anticipate barriers to communication and take action to improve communication.
• Maintain effective communication with senior clinical colleagues, the wider team, the organisation and with external stakeholders.
• Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Non-Medical Prescribing

• You must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes.
• If you are not a NMP, you will work towards this qualification.
• You must maintain your knowledge and skills and prescribe within your clinical competencies.
• The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance.

Delivering a Quality Service

• Recognise and work within own competence and professional code of conduct as regulated by the professional regulatory body. Understand own role and scope and identify how this may develop over time.
• Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

• Ensure professional revalidation requirements are met.
• Deliver care according to NSF, NICE guidelines and evidence-based care.
• Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.

• In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
• Support and participate in shared learning across the organisation and wider organisation.
• Understand and apply guidance that supports the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patient health procedures and local guidance.
