Job overview

Under the direction of the Lead Care of Older People and Stroke Pharmacist in conjunction with the Pharmacy Lead for Clinical Services at Northwick Park Hospital, the post holder will:

  • *Provide an enhanced clinical pharmacy service to the Same Day Emergency Care (SDEC)-Frailty (Northwick Park), Stroke and Older People wards, including Pharmacy Integrated Care Service (PICS)
  • *SDEC-Frailty Pharmacy service is a referral based enhanced service for complex medicines optimisation and review
  • *To work as part of the Stroke and Older People teams and liaise with community-based services to ensure the seamless transfer of patient care on discharge.
  • *Provide daily ward pharmacy service to Care of Older people wards and SDEC-Frailty team when referrals are made

Main duties of the job

  • To lead and develop the specialist pharmacy service to the acute geriatric wards (including stroke) and services (SDEC-Frailty) and where applicable, under the direction of Lead Pharmacist for Older People and Stroke for the Trust, be involved in Virtual Ward and /or Hospital@Home service, when set up.
  • To provide highly specialised advice on all medicine related issues in area of responsibility i.e, cost effective optimal treatment either for individual patients or groups of patients in line with local, regional and national guidance and policies
  • Provides high level of pharmaceutical care to patients on the ward, obtaining accurate medication history, reviewing choice of treatment and dosage to ensure safe and effective use of medicines in individual patients in line with Trust guidelines and Formulary. Using Person Centred Care, to support patients in decision making involving their care, and what is important to them.

Person specification

Experience

Essential

  • Relevant Older people & Stroke hospital experience
  • Wide range of ward pharmacy experience
  • Previous relevant specialist clinical experience e.g. emergency, acute or older people medicine
  • Experience in multidisciplinary team working
  • Demonstrate up to date knowledge of treatment guidelines

Desirable

  • Experience of working in frailty and ambulatory care units
  • Evidence of evaluating own and pharmacy staff’s work to drive and deliver improvements in service quality

Qualifications

Essential

  • Registered as a Pharmacist with the General Pharmaceutical Council
  • Diploma in Pharmacy Practice
  • Non-Medical Prescriber
  • Frailty Course

Desirable

  • MSc in Clinical Pharmacy Practice
  • Evidence of formal management training
