Job overview

This post is a middle grade position which will experience in a supported Emergency Medicine department. The applicant should be aiming to gain more experience before entering formal training at ST4 onwards or by the CESR route and Article 14, or the Defined Route of Entry into Emergency Medicine (DREEM).

This is a new middle grade position as Senior Clinical Fellow in Emergency Medicine with the potential to consolidate their experience in Emergency Medicine. Alongside developing their clinical skills the post will allow the applicant to develop Ultrasound and Research skills. This department has a strong link to ultrasound training and teaching and will provide support to interested candidates to achieve Level 1 & Level 2 Ultrasound skills.

This post will also allow you to take part in the active research and quality improvement programs within the department. Many of the research projects have been presented in international conferences and also published.

Main duties of the job

  • You will be part of the multidisciplinary clinical team as middle grade in the Emergency Department.
  • Provide high quality care, knowledge, and skill for the patients either directly or by supervision and floor management of the junior clinical staff
  • To maintain the standards of the College of Emergency Medicine and the West Suffolk Hospital NHS Foundation Trust
  • To assume clinical responsibility for patients brought to the department by the emergency services, self-referring or referred by GP
  • You are expected to deal with unexpected emergencies including those resulting from infectious diseases like COVID-19
  • To be part of Trauma Team co-ordinated management of major trauma
  • To be immediately available for opinion, guidance and advice on any patient seen by a junior clinician in the ED at any time during the rostered duty shift

Detailed job description and main responsibilities

The children requiring emergency care in the West Suffolk area are lucky to have a dedicated 24/7 children's area within the main emergency department. This area is mainly self-contained and has its own triage room, 6 trolley spaces (split into 2 bays) and its own waiting area, which is situated near to the ED x-ray and allows Children to been seen and treated with minimal contact with adult patients. The area also has its own dedicated facilities such as toilets, a kitchen area, sluice, drug prep area, and triage room.

Currently we have 3 consultant paediatricians working clinical PAs in ED to help support with the clinical education of the junior doctors and the treatment of patients. We currently have funding for three paediatric nurses per shift and are actively recruiting to fill some vacancies, as recently some staff nurses have progressed to more senior posts and to become trainee ENPs.

The paediatric nursing team includes a band 7 lead, band 6 nurses and experienced band 5 nurses, all of whom have extensive clinical experience in the assessment and treatment of children who require emergency care. The paediatric nurses are all EPALS provider trained, and the team also have one EPALS and one PILS instructor among them who work with the resuscitation team to deliver training across the trust.

Eligibilty

Essential

  • Full registration with the GMC and hold a Licence to Practise at time of starting the post.
  • Minimum of 5 years postgraduate medical experience, of which at least 2 years should be in Emergency Medicine, including at least 6 months working in UK Emergency Medicine at a level equivalent to ST4 or above.
  • Evidence of achievement of foundation competencies and of clinical competence at ST3 / core-training level or above.
  • Level 3 safeguarding children training.

Desirable

  • Experience of working at registrar (ST3+) level in a UK major trauma centre or paediatric major trauma centre.
  • Six months dedicated experience in paediatric emergency medicine

Qualifications and Experience

Essential

  • MBBS or equivalent medical qualification
  • MRCEM Primary and MRCEM SBA passed, or equivalent / legacy qualification (e.g. FRCEM Primary and Intermediate). Candidates working towards, or who already hold, full MRCEM are encouraged to apply.

Desirable

  • Primary FRCEM
  • Relevant postgraduate qualification (e.g. PGCert Medical Education, MSc, RCEM-accredited ultrasound).

Acute Care (Knowledge and Skills)

Essential

  • Ability to assess, recognise and initiate management of the acutely ill patient.
  • Able to lead a resuscitation and contribute effectively to the trauma team.
  • Current ALS provider certificate. ATLS/ETC and APLS/EPLS in date, or willingness to obtain within an agreed time frame.

Desirable

  • All three life support courses (ALS, ATLS and APLS/EPLS) in date; instructor status in any of these.
  • Trauma team experience
  • RCEM core ultrasound or equivalent (FUSIC/FAMUS)

Clinical care (knowledge and skills)

Essential

  • Competent in the core Emergency Medicine procedural skills expected at ST3+ level.
  • Able to independently assess and manage undifferentiated patients across all areas of the ED, and to make safe shop-floor decisions with consultant support available.
  • Aware of own limitations and knows when to consult senior colleagues.
  • Understands the importance of working effectively in teams.
  • Able to help manage the clinical floor, prioritise the activities of the team, and supervise and support junior clinical staff.

Desirable

  • Knowledge and understanding of UK health systems.
  • Awareness of current issues in the NHS

Communication and Interpersonal Skills

  • Able to communicate effectively in written English.
  • Able to communicate effectively in spoken English to enable effective communication with patients and colleagues.
