JOB SUMMARY

The Support Worker will be a member of the Day Centre Team providing Day Opportunities to Adults with physical disabilities using a person centred approach. The Support Worker will provide personal care and manage challenging behaviour when required. Staff within the Day Centre are expected at all times to provide a caring service and to treat those with whom they come into contact in a courteous and respectful manner.

KEY DUTIES / RESPONSIBILITIES

Provide personal assistance with primary needs i.e. toileting, feeding, bathing, showering.

Facilitate and enable service users to participate in activities.

Be responsible for the care of a group of service users in the absence of the Day Care Worker.

Implement agreed individual programmes of support designed to enhance and maximise the capabilities of adults with a disability (eg Behaviour Management Programmes Support regarding personal hygiene skills, life skills, employment skills and promote enhancement of self esteem).

Implement social and recreational programmes.

Implement appropriate individual behavioural strategies identified by Behaviour support Team.

Assist with the management and monitoring service users’ personal finances

Participate in meeting the physical and personal needs of service users.

Meet the needs of service users with complex needs or challenging behaviours and be willing to be trained to facilitate this.

Assist with the management and administration of medication on completion of training.

Participate in the assessment, monitoring and review for each client.

Assess risks to clients, staff and members of the public and report to the Manager.

Liaise regularly with other Day Care Staff regarding their input with the client / carer.

Co-work within the existing key worker system and provide support to other Day Care Staff as required.

Act as bus escort as and when required to convey clients to and from their place of residence.

Be required to work across settings to encourage independence and promote community participation.

Participate in a formal programme of supervision, staff performance and development review.

Contribute to and participate in training opportunities relevant to their post

Provide and maintain appropriate case records and reports to support the monitoring and reviewing of programmes of support.

Observe confidentiality in respect of his / her exchanges with service users / carers.

Perform their duties in accordance with the South Eastern Health and Social Care Trust Policies and Procedures and in adherence to legislation.

GENERAL MANAGEMENT RESPONSIBILITIES
