JOB TITLE Specialist Physiotherapist – Hospital at Home

ACCOUNTABLE TO: Lead for Hospital at Home / Physiotherapy Lead for Intermediate Care
PROFESSIONALLY RESPONSIBLE TO: Head of Physiotherapy
JOB SUMMARY
To provide a high standard Specialist physiotherapy service based on accurate diagnosis and prognosis to develop and deliver treatment programmes designed to meet each individual patient’s specific needs.
To be professionally responsible to the Head of Physiotherapy in SEHSCT.
To work as an autonomous practitioner within the multidisciplinary team, treating acutely unwell patients in their own home.
To liaise with and appropriately sign post to other community teams in the effective and safe management of patients within their own home.
To participate in and action evidence-based audit and research projects to further own and team’s clinical practice.
To assist with the implementation of strategic development of this service including specific changes to service protocols/policies/procedures.
To co-operate in the supervision and education of self, other physiotherapists, members of multi-disciplinary teams, students, as well as more junior physiotherapy staff.
KEY DUTIES / RESPONSIBILITIES
1. To ensure that own practice and that of staff under your supervision is in keeping with the Physiotherapy Code of Professional Values and Behaviour and meets the required professional, national and local standards of physiotherapy practice
2. The post holder will be expected to ensure that all clinical practice is based on current
research/evidence and must demonstrate a sound knowledge of clinical governance and
risk management to ensure delivery of a safe high quality service.
3. To undertake comprehensive assessment and treatment and keep satisfactory up to date records of assessments and treatments carried out while ensuring adequate discharge arrangements are in place.
4. To represent physiotherapy service and/or individual patients at multidisciplinary team
meetings, to ensure the delivery of a co-ordinated multidisciplinary service, and integrate
physiotherapy treatment into the treatment programme. This will include discussion of
patient care, patient progress and involvement in discharge planning.
5. To be responsible for own practice and accountable for all aspects of physiotherapy
treatment and care delivered to the patient.
6. Assess patient understanding of treatment proposals, gain valid informed consent and
work within a legal framework with patients who lack capacity to consent to treatment.
7. To be responsible for maintaining accurate, timely and comprehensive patient treatment records in line with the Trust and Chartered Society of Physiotherapy (CSP) standards of practice. Supervise junior and student physiotherapists’ record keeping system according to professional physiotherapy service standards.
8. Demonstrate the ability to discuss sensitive information i.e. deal sensitively with families who have a high level of anxiety e.g. accepting rehabilitation versus care needs.
9. To support the assessment, diagnosis and development of individual care/treatment plans for patients with respiratory conditions, some of which will be highly complex or will be for patients with complex clinical, social and psychiatric needs.
10. To be able to critically reason, analyse clinical and social information from a range of
sources and integrate into physiotherapy assessment and development of treatment plan.
11. To carry out risk assessment with patient and carers in the community and to advise/instruct carers appropriately.
12. To provide other professionals involved in the care of your patients with advice in
relation to appropriate physiotherapy treatments. This includes providing advice to members of the multidisciplinary team regarding patients’ respiratory function, condition and physiotherapeutic management of the same.
14. To take delegated responsibility from the Band 7 physiotherapist for managing patients with respiratory conditions, elderly and neurological rehab and patients with a physical disability.
15. Use a range of communication tools to communicate complex information effectively with patients re diagnosis and prognosis alongside providing encouragement and motivation. This includes patients who have difficulties in communication and understanding e.g. dysphasia, deafness, cognitive impairment, and where English is not the first language.
16. Make use of a range of behavioural change skills to motivate patients to maximise their rehabilitation potential, promote a degree of self-management, and make informed lifestyle changes to improve health related quality of life.
Organisational
1. To deputise for more senior staff in their absence, taking responsibility for allocating and organising physiotherapy work to meet service and patient priorities, readjusting plans as the situations change/arise.
2. Ensure safe practice under the Health & Safety at Work order. Ensure that equipment used by the team is safe and maintained at all times.
3. Take responsibility for assessment and ordering of specialised equipment and updating knowledge of availability of new equipment. Training of patients and carers re safe use of this equipment.
4. To attend meetings as required by line manager.
5. To ensure effective communication with all members of the uni-professional and multidisciplinary teams.
6. To closely liaise with all Senior Managers in order to effectively discharge the stated duties of the post.
7. Be required to work flexibly to meet the needs of the service across all Trust localities, evenings and / or week ends
Professional Development
1. Facilitate CPD, for self and more junior staff and ensure accurate records and reflective learning recorded.
2. To be responsible for maintaining own competency to practice through CPD and maintain a portfolio which reflects personal development.
3. Provide clinical supervision, training, mentoring and advice to qualified and non-qualified staff.
4. To be responsible for maintaining own competency to practice in line with HCPC
regulations, KSF profile and Personal Development Plan.
5. Be an active member of the in-service training programme by attendance at and
participation in, in-service training programmes, tutorials, individual training sessions,
external courses and peer review.
6. To participate in the induction of new staff and to encourage them to further their
professional education in line with the Departmental CPD Strategy.
7. To supervise junior staff and assistants and delegate rehabilitation programs to support workers in line with their level of competence and expertise.
8. To be an active member of the in-service training programme by attendance at and
9. To supervise, educate and assess the performance of physiotherapy students; this would be to a graduate standard and involve working with universities to ensure the standard of practice and teaching meets the standards set by the degree level qualification.
10. Maintain and develop current knowledge and implementation of evidence-based practice in the care of respiratory conditions and management of frail patients.
Service Improvement
1. To co-operate and assist in the physiotherapy emergency out of hours rota.
2. To participate in clinical audit and research and undertake evidence-based projects to
assist in improvements to service delivery and clinical practice.
3. To continually review the therapy techniques/treatments used in light of evidence based practice and research findings and evaluate their effectiveness.
4. To support the specialist respiratory physiotherapist and the wider MDT in the
development of policies and procedures for care of respiratory patients. To provide statistical information, as required, helping monitor and develop the service.
5. Maintain accurate information in relation to the Trust’s Information Systems.
6. To assist develop a more flexible workforce better able to respond to the changing needs of the Service.
7. Be actively involved in professional and relative clinical interest groups
