JOB SUMMARY The Support Worker will be responsible for delivering support to individuals whose illness impacts on their ability to maintain their home and independence in the community setting. The role of 53 Ardglass Road Supported Living Unit is to use its specialist knowledge, experience, skills and facilities in close collaboration with service users, carers and other professionals to create an individual and personal goal-centred, Support Plan. By collaborative, interventions, we advise, support and seek to empower service users helping them achieve independence, and preserve their identity, culture and strengths using a recovery focused model of support.
To provide advice, support and assistance where necessary to help service users achieve the short and long-term goals as indicated in Individual and Personal Support Plans.
To support service users in all areas indicated by their Support Plan using a Recovery Focused approach designed to maximise their potential and promote their independence and maintain their home.
To facilitate, encourage and enable service users to participate in planned Social, Occupational, and Rehabilitative activities using a person-centred Recovery focused approach.
To implement agreed individual programmes of support and activity designed to enhance and maximise the independence of our service users (as agreed with Service Users, Support Workers, Carers, Families and the Multi- Disciplinary Team.
This will include all areas of Support Plans ie; Social and Domestic Skills, Home Management (cooking, cleaning, laundry etc,) and Budgeting. To administer and manage medications as identified in the tenant Support Plan. This is not a definitive list and all areas will be agreed with designated Manager and have Multi- disciplinary team input.
To implement individual recovery focused strategies identified by the Multi- disciplinary team.
To participate in the assessment monitoring and review for each service user.
To assess risks to service users, staff and members of the public and to effectively report all concerns to the Manager or to the service users designated Keyworker
Support Workers will liaise regularly with the Manager and other members of the Multi-disciplinary team regarding their input with service users.
He/she will support tenants with identified difficulties in successfully organising their travel requirements to meet the requirements identified in their individual plans.
He/she will work successfully both independently and with existing guidance. All staff are expected to provide support to their colleagues and to work effectively within the team, to achieve the best possible outcomes for service users, carers and staff.
He/she will work within an existing 24 hour rota system in accordance with Trust Policy.
He/she will engage in escort duties as and when required to support service users to and from their place of residence, to appointments, engagements etc.
He/she will be required to work across settings to encourage independence and promote community participation and social integration.
He/she will participate in a formal programme of supervision, staff performance and development review.
He/she will contribute to and participate in training opportunities relevant to their post
He/she will provide and maintain appropriate case records and reports to support the monitoring and reviewing of programmes of support.
He/she will observe “confidentiality” in respect of his/her exchanges with service-users/carers.
The Support worker will support and encourage service users to engage with their community and will help service users realise their potential and involvement within their local community.
He/she will perform their duties in accordance with the South Eastern Health and Social Care Trust Policies and Procedures and in adherence with current Data Protection legislation.
The post holder will promote and support effective team working, fostering a culture of openness and transparency.
