JOB SUMMARY

In order to meet the overall mission of the trust the post holder will

Work as part of the Community Children’s Nursing team to provide care for children within the community setting as delegated by the Registered Children’s Nurse. They will provide care to children and young people including those with complex health needs who are dependent on others to meet their care needs.

Work in partnership with families whilst providing care.

Promote independence and accept responsibility for activities agreed and allocated by the line manager

The range of duties the post holder will undertake will be determined by the training competencies of the post holder and the individual child’s needs, as assessed and delegated by the Registered Children’s Nurse.

KEY DUTIES / RESPONSIBILITIES

Operational Delivery

Work as an integral member of the Community Children’s Nursing Team, carrying out duties within their assessed competencies as agreed and delegated by the Registered Children’s Nurse using a person centred approach to the delivery of all care

Be trained and assessed as competent in carrying out observations on children and young people and accurately document same

Promote, monitor and maintain health, safety and security in the working environment at all times

Under the direction and delegated by Registered Children’s Nurse, and following child specific training, the post holder will carry out tasks/procedures that are detailed in each child/young person’s individualised care plan. This may include supporting prescribed enteral feeding, administration of medicines, airway management, seizure management including administration of emergency rescue medication; attend to personal care needs and assist in the management of continence bladder and bowel

Ensure supplies are adequate and that home nursing equipment is appropriately used and maintained in a safe condition

Information Management;

Report changes in child/young person’s circumstances or condition, as well as all untoward incidents/accidents to Registered Nurse and parent/carer with parental responsibility.
