JOB SUMMARY

In order to meet the overall mission of the Trust the post holder will

Work as an Aseptic Services Pharmacist providing a full range of aseptic dispensing services to wards and departments within SE trust.

Work as a Clinical Pharmacist providing a clinical pharmacy service to oncology/ haematology inpatients and outpatients and to patients requiring Parenteral Nutrition within the trust.

Ensure the safe, effective and economic use of medicines.

KEY DUTIES / RESPONSIBILITIES

To practise as an Independent Pharmacist Prescriber within the area of oncology/haematology or PN. Required to undertake and attain the prescribing qualification within first 2 years of appointment.

Undertake the NHS Technical Specialist Education and Training group (NHS TSET), Aseptic Preparation and Dispensing of Medicines (APDM) course within the first year of employment

Provide a high level of clinical pharmacy input, as part of the multi-disciplinary team.

Develop good working relationships with the other members of the healthcare team, negotiating and influencing changes in prescribing practice to promote evidence based practice.

Participate in the provision and development of the full range of aseptic dispensing services, including cytotoxic preparation service, TPN and CIVAS services

Maintain competency within the area of aseptic services to enable inclusion in register of designated personnel for intrathecal chemotherapy

Enusre robust processes are in place for the quarantine and release of unlicensed medicines, particularly externally procured ready to administer products, and maintain compliance with regulatory requirements.

Lead the development, monitoring and reporting of Key Performance Indicators (KPIs), capacity planning and error reporting and trending.

Support the management and continuous improvement of the PQS, working alongside the Lead PQS Technician, to ensure all standards are fully implemented. This includes ownership and accountability for quality management processes and delivery of associated actions.

Review and approve change control requests and operational documentation

Undertake regular audits as part of the PQS, working alongside the Accountable Pharmacist to implement any recommendations.

Obtain a comprehensive and accurate admission medication history to ensure that patients receive the correct and appropriate medicine in a timely manner.

Implement pharmaceutical care plans in accordance with patients’ diagnosis and assess treatment options available.

Provide a prescription monitoring service including identification of adverse drug reactions, drug related events, drug interactions and rationalisation of medicines during hospital stay.

Provide a clinical prescription monitoring service for all chemotherapy, PN and other aseptic prescriptions including identification of appropriateness of treatment, relevant acceptable test results, drug interactions, drug/disease interactions.

Provide medication counselling and education to patients and carers during hospital stay, at discharge and during attendance at cancer unit.

Ensure timely discharge information is available to primary care healthcare professionals.

Ensure timely information regarding chemotherapy regimens and side effects for outpatients attending cancer unit, is available to primary healthcare professionals.

Provide medicine related information and advice of a therapeutic, legal or procedural nature to prescribers and other healthcare staff ensuring effective prescribing and patient safety at all times.

Help develop, implement and monitor regional and local policies, protocols, guidelines and initiatives in the area of medicines management.

Undertake risk assessments to ensure any potential medication incidents are minimised.

Assist in benchmarking the clinical pharmacy and aseptic service and contribute to the development of performance indicators to monitor effectiveness and patient benefit.

Promote optimal, safe and appropriate drug use and high quality prescribing practice through development of local formularies, drug usage review, development of prescribing guidelines and multidisciplinary staff training.

To participate in Medical, Nursing and allied professional education as appropriate.

Carry out intervention monitoring according to local guidelines to ensure the safe, effective and economic use of medicines.

Attend multidisciplinary meetings as appropriate and provide timely analysed drug usage reports with recommendations on budgetary and formulary activities.

Proactively contribute to therapeutic decisions concerning patient management.

Advise the Clinical Directors and Head of Pharmacy and Medicines Management of budgetary changes due to possible alterations in the use of drugs e.g. new licensed indications, new literature and new drugs marketed.

Initiate and participate in pharmaceutical aspects of clinical audit, research and supervise students where appropriate.

To participate in the planning and administration of drug trials.

Participate in relevant Continuing Professional Development (CPD) and apply knowledge gained to current and future work as appropriate.

Collate and monitor information on the expenditure of high cost drugs to ensure cost effective prescribing.

Participate in the mentoring, teaching and training of band 6 pharmacists on the Foundation Programme, pre-registration pharmacists and university students.

Participate in the mentoring and teaching of pharmacy technicians on the Medicines Management Accredited Programme (MMAP) and Aseptic Pre and In Process (PIP) Checking Programme

Supervise and support the junior pharmacists and technical staff working within your area on a daily basis

Ensure the appropriate interpretation of prescriptions prior to their preparation.

Provide accurate and appropriate medicines information in response to internal and external Trust enquiries including patient/carer/healthcare worker enquiries utilising all available resources.

Plan and organise own work according to agreed objectives and timeframes for the designated area/pharmacy service.

Collect and collate all workload stats.

Report incidents and near misses in line with Trust policy

Encourage staff to report near misses and incidents, share the learning and undertake reflective practice

Encourage pharmacists, medical and nursing staff to report adverse drug reactions

Participate in the preparation of conference presentations, posters, writing reports and the writing up of publications.

Participate in the implementation of policies and procedures for the safe and efficient use of medicines/Pharmaceutical Clinical Effectiveness Programme.

Participate in the recruitment, selection and appointment of staff as required

Ensure the safe and secure storage of medicines in accordance with all medicines legislation and Trust security policies.

Generic managerial responsibilities both within an operational and strategic

context.

Maintain staff relationships and morale amongst staff

Supervise and assist in the training of Foundation Year Trainee Pharmacists, Technicians, Student Technicians and Assistant Technical Officers.

Reports critical incidents/near misses using the Trust reporting mechanism

Report any signs of ill health/stress in colleagues

Support the Trust in defending legal actions and participate in tribunals as required

Participate in the opening/lock up, weekend and statutory duty rota and when required, on the on call rota.

Comply with all Trust policies and procedures relevant to area of work

Support the Trust in its drive for seven day working in order to ensure it best meets the needs of our patients.

To progress to band 7 candidates must complete and pass NHS Technical Specialist Education and Training group (NHS TSET) and Aseptic Preparation and Dispensing of Medicines (APDM) course

Those submitting FP1/FP2 in 2026 are eligible to apply for this post and will progress to band 7 on completion.

Other

Hold a current full driving licence which is valid for use in the UK and have access to a car on appointment.

Able to attend the Ulster Pharmacy within 45 minutes if called to attend whilst on call.

This criteria will be waived in the case of applicants whose disability prohibits driving but who have access to a form of transport approved by the Trust which will permit them to carry out the duties of the post.
