An exciting opportunity has arisen for a General Practitioner (GP) to join the Northern Care Alliance at  the Salford Urgent Response Team on a fixed-term or secondment basis.

The postholder will play a key role in supporting the ongoing development of the step up services within Salford Urgent Response service, aligned with Northern Care Alliance (NCA) improvement programmes and strategic priorities. The role will contribute to delivering a sustainable, high-quality model of urgent community care that effectively manages demand while meeting national guidance, reduce admission, and quality standards.

Main duties of the job

  • Provide senior clinical assessment, decision-making, and management for patients referred to the Urgent Response Team.
  • Support rapid assessment and intervention for patients experiencing acute health deterioration, helping them remain safely at home where appropriate.
  • Lead clinical decision-making within the Step-Up Pathway to provide alternatives to hospital admission.
  • Work collaboratively with community multidisciplinary teams, including nurses, therapists, pharmacists, social care colleagues, and specialist services.
  • upport the continued development and transformation of the Salford Urgent Response service.
  • Lead and support the development of the Step-Up Pathway, providing alternatives to hospital admission through rapid community-based assessment and intervention.
  • Strengthen collaborative working and build effective relationships with GP practices, Primary Care Networks, community services, and wider system partners.
  • Provide clinical leadership to ensure safe, effective, and patient-centred urgent care delivery.
  • Contribute to service improvement initiatives that enhance quality, efficiency, and patient outcomes.
  • Support the delivery of national urgent care standards and local performance objectives.
  • Promote integrated working across health and social care services to improve patient pathways and experiences.
